Radium-223 dichloride - Xofigo®/Alpharadin®

According to the NCI website radium Ra 223 dichloride - Xofigo®/Alpharadin® is a radiopharmaceutical composed of the dichloride salt of the alpha-emitting isotope radium Ra 223, with antineoplastic activity. Like calcium, radium targets bone tissue and preferentially accumulates in osteoblastic lesions, such as those seen in areas of bone metastases. Radium Ra 223 forms complexes with hydroxyapatite and becomes incorporated into the bone matrix. The radioisotope Ra 223 kills bone cancer cells through local emission of high energy alpha particles, causing DNA double-strand breaks and tumor regression in the skeleton. The short range effects of alpha emission allows for localized DNA damage with limited toxicity to nearby healthy bone tissue.

Indikationen/Anwendungsmöglichkeiten gemäss Fachinformation des Arzneimittel-Kompendium der Schweiz®:

  • Xofigo ist indiziert für die Behandlung von Patienten mit kastrationsresistentem Prostatakarzinom (CRPC) und symptomatischen Knochenmetastasen ohne bekannte viszerale Metastasen. Bei nicht orchiektomierten Patienten soll eine antiandrogene Behandlung zur Suppression des Testosteronspiegels auf Kastrationsniveau weitergeführt werden.

 

 

Raloxifene / Evista®

According to the NCI website raloxifene (Evista®) is a selective benzothiophene estrogen receptor modulator (SERM). Raloxifene binds to estrogen receptors (ER) as a mixed estrogen agonist/antagonist; it displays both an ER-alpha-selective partial agonist/antagonist effect and a pure ER-beta-selective antagonist effect. This agent functions as an estrogen agonist in some tissues (bones, lipid metabolism) and as an estrogen antagonist in others (endometrium and breasts), with the potential for producing some of estrogen's beneficial effects without producing its adverse effects.

Belegte Indikationen
Evista ist angezeigt zur Behandlung und Prävention der Osteoporose bei postmenopausalen Frauen.
Voraussetzung für die Einleitung einer Präventionsbehandlung der postmenopausalen Osteoporose ist ein densitometrisch (DXA) festgehaltener T-Wert von mindestens –1 an der Wirbelsäule oder am distalen Vorderarm.
Bei der Behandlung der Osteoporose mit Evista wird die Inzidenz vertebraler Frakturen signifikant reduziert. Die Reduktion der Inzidenz nicht-vertebraler Frakturen ist nicht belegt.
Wenn bei einer postmenopausalen Frau eine Entscheidung zwischen Evista und anderen Therapiemöglichkeiten (Hormonersatztherapie) getroffen werden soll, sind im individuellen Fall klimakterische Symptome, Auswirkungen auf das Uterus- und Brustgewebe sowie kardiovaskuläre Risiken und Nutzen zu berücksichtigen.

Ramucirumab/Cyramza®

According to the NCI website ramucirumab is a recombinant, fully human monoclonal antibody directed against human vascular endothelial growth factor receptor 2 (VEGFR-2) with antiangiogenesis activity. Ramucirumab specifically binds to and inhibits VEGFR-2, which may result in an in inhibition of tumor angiogenesis and a decrease in tumor nutrient supply.

Magenkarzinom:

Cyramza ist indiziert in Kombination mit Paclitaxel für die Behandlung von erwachsenen Patienten mit fortgeschrittenem Adenokarzinom des Magens oder gastroösophagealen Übergangs mit einem Progress nach vorausgegangener Platin- und Fluoropyrimidin-haltiger Chemotherapie.

Cyramza Monotherapie ist inzidiert für die Behandlung von erwachsenen Patienten mit fortgeschrittenem Adenokarzinom des Magens oder des gastroösophagealen Übergangs mit einem Progress nach vorausgegangener Platin- oder Fluoropyrimidin-haltiger Chemotherapie, wenn diese Patienten für eine Kombinationstherapie mit Paclitaxel nicht geeignet sind.

Raltitrexed / Tomudex®

According to the NCI website raltitrexed (Tomudex®) is a quinazoline folate analogue with antineoplastic activity. After transport into cells via the reduced folate carrier, raltitrexed undergoes intracellular polyglutamation and blocks the folate-binding site of thymidylate synthase, thereby inhibiting tetrahydrofolate activity and DNA replication and repair and resulting in cytotoxicity.

Indikationen/Anwendungsmöglichkeiten gemäss Arzneimittel-Kompendium der Schweiz®:

Palliative Behandlung des fortgeschrittenen Kolorektalkarzinoms, wenn eine 5-FU/Leucovorin-Therapie nicht in Frage kommt. Aus klinischen Studien liegen keine Erfahrungen mit Patienten mit WHO Performance-Status 3 und 4 vor.

Antimetaboliten

Regorafenib - STIVARGA®

Regorafenib - Stivarga® ist ein oral bioverfügbares Kleinmolekül mit potentiellen antiangiogenen und antineoplastischen Aktivitäten. Regorafenib bindet an und hemmt die vaskulären endothelialen Wachstumsfaktor-Rezeptoren (VEGFRs) 2 und 3 und die Ret-, Kit-, PDGFR- und Raf-Kinasen, was zur Hemmung der Tumor-Angiogenese und Tumorzellproliferation führen kann. VEGFRs sind Rezeptor-Tyrosin-Kinasen, die eine wichtige Rolle bei der Tumorangiogenese spielen; die Rezeptor-Tyrosin-Kinasen RET, KIT und PDGFR und die Serin/Threonin-spezifischen Raf-Kinasen sind an der Tumorzellsignalisierung beteiligt.

Indikationen/Anwendungsmöglichkeiten gemäss Compendium®:

  • Stivarga ist indiziert zur Behandlung von Patienten mit metastasiertem kolorektalem Karzinom (CRC), die zuvor eine fluoropyrimidin-, oxaliplatin- und irinotecanbasierte Chemotherapie, eine anti-VEGF-Therapie und – bei Vorliegen eines RAS-Wildtyps – eine anti-EGFR-Therapie erhalten haben.
  • Stivarga ist indiziert zur Behandlung von Patienten mit metastasierten oder inoperablen gastrointestinalen Stromatumoren (GIST), die zuvor mit zwei Tyrosinkinase Inhibitoren (Imatinib und Sunitinib) behandelt wurden.
  • Stivarga ist indiziert zur Behandlung von Patienten mit Leberzellkarzinom (HCC), die zuvor mit Sorafenib behandelt wurden.

Ribociclib - KISQALI®

According to the NCI website Ribociclib is an orally available cyclin-dependent kinase (CDK) inhibitor targeting cyclin D1/CDK4 and cyclin D3/CDK6 cell cycle pathway, with potential antineoplastic activity. CDK4/6 inhibitor LEE011 specifically inhibits CDK4 and 6, thereby inhibiting retinoblastoma (Rb) protein phosphorylation. Inhibition of Rb phosphorylation prevents CDK-mediated G1-S phase transition, thereby arresting the cell cycle in the G1 phase, suppressing DNA synthesis and inhibiting cancer cell growth. Overexpression of CDK4/6, as seen in certain types of cancer, causes cell cycle deregulation.

 

Indikationen gemäss Compendium®:

Kisqali ist indiziert bei Hormon-Rezeptor (HR)-positivem, humanen epidermalen Wachstumsfaktor-Rezeptor-2 (HER2)-negativem fortgeschrittenem oder metastasierendem Mammakarzinom in Kombination:

  • mit einem Aromatasehemmer als anfängliche endokrine Therapie zur Behandlung prä-, peri-oder postmenopausaler Frauen.
  • mit Fulvestrant als anfängliche oder nach einer vorherigen endokrinen Therapie zur Behandlung postmenopausaler Frauen.

Bei prä- oder perimenopausalen Frauen sollte die endokrine Therapie mit einem Agonisten des luteinisierenden Hormon-Releasing-Hormons (LHRH) kombiniert werden.

Rigosertib

 

According to the NCI website, rigosertib sodium is a synthetic benzyl styryl sulfone analogue with potential antineoplastic activity. Rigosertib sodium inhibits polo-like kinase1 (Plk1), inducing selective G2/M arrest followed by apoptosis in a variety of tumor cells while causing reversible cell arrest at the G1 and G2 stage without apoptosis in normal cells. This agent may exhibit synergistic antitumor activity in combination with other chemotherapeutic agents. Plk1, named after the polo gene of Drosophila melanogaster, is a serine/threonine protein kinase involved in regulating mitotic spindle function in a non-ATP competitive manner. Behandlung von Patienten mit Paget-Krankheit der Knochen.
Voraussetzung für die Einleitung einer Präventionsbehandlung der postmenopausalen Osteoporose ist ein densitometrisch (DXA) festgehaltener T-Wert von ≤-1 an der Wirbelsäule oder am distalen Vorderarm, d.h. eine Reduktion der Bone Mineral Density (BMD) um mindestens eine Standardabweichung des Referenzwertes für eine Normalpopulation. Eine Behandlung der Osteoporose wird ab einem T-Wert von ≤-2,5 oder dem Vorliegen manifester atraumatischer Frakturen eingeleitet.

Bisphosphonat

Ripretinib – Qinlock (USA)

According to the NCI website, ripretinib is an orally bioavailable switch pocket control inhibitor of wild-type and mutated forms of the tumor-associated antigens (TAA) mast/stem cell factor receptor (SCFR) KIT and platelet-derived growth factor receptor alpha (PDGFR-alpha; PDGFRa), with potential antineoplastic activity. Upon oral administration, ripretinib targets and binds to both wild-type and mutant forms of KIT and PDGFRa specifically at their switch pocket binding sites, thereby preventing the switch from inactive to active conformations of these kinases and inactivating their wild-type and mutant forms. This abrogates KIT/PDGFRa-mediated tumor cell signaling and prevents proliferation in KIT/PDGFRa-driven cancers. DCC-2618 also inhibits several other kinases, including vascular endothelial growth factor receptor type 2 (VEGFR2; KDR), angiopoietin-1 receptor (TIE2; TEK), PDGFR-beta and macrophage colony-stimulating factor 1 receptor (FMS; CSF1R), thereby further inhibiting tumor cell growth. KIT and PDGFRa are tyrosine kinase receptors that are upregulated or mutated in a variety of cancer cell types; mutated forms play a key role in the regulation of tumor cell proliferation and resistance to chemotherapy.   • Gastrointestinal stromal tumor that is advanced. It is used in adults whose cancer has been treated with at least three kinase inhibitors, including imatinib mesylate.

Rituximab / MabThera®

According to the NCI website Imatinib Palbociclib is an orally available cyclin-dependent kinase (CDK) inhibitor with potential antineoplastic activity. Palbociclib selectively inhibits cyclin-dependent kinase 4 (CDK4) and 6 (CDK6), thereby inhibiting retinoblastoma (Rb) protein phosphorylation early in the G1 phase leading to cell cycle arrest. This suppresses DNA replication and decreases tumor cell proliferation. CDK4 and 6 are serine/threonine kinases that are upregulated in many tumor cell types and play a key role in the regulation of cell cycle progression.

Indikationen/Anwendungsmöglichkeiten gemäss Arzneimittel-Kompendium der Schweiz®:

Non-Hodgkin-Lymphom

Monotherapie bei Patienten mit CD20 positivem follikulärem Non-Hodgkin-Lymphom (Stadien III-IV), bei Rezidiv nach oder Nichtansprechen auf Chemotherapie.

Behandlung von vorgängig unbehandelten Patienten mit CD20 positivem follikulärem Non-Hodgkin-Lymphom (Stadien III-IV) mit hohem Tumorload in Kombination mit CVP oder CHOP. Bei Ansprechen auf die Therapie kann eine Erhaltungstherapie mit Rituximab Monotherapie über 2 Jahre verabreicht werden.

Erhaltungstherapie von Patienten mit CD20 positivem rezidiviertem oder refraktärem follikulärem Non-Hodgkin-Lymphom (Stadien III-IV), die auf eine Induktionstherapie mit CHOP mit oder ohne Rituximab angesprochen haben.

Behandlung von Patienten mit einem CD20 positiven diffusen grosszelligen B-Zell-Non-Hodgkin-Lymphom (DLBCL) in Kombination mit Standard-CHOP (8 Zyklen Cyclophosphamid, Doxorubicin, Vincristin, Prednison).

Behandlung von Patienten mit therapiebedürftiger chronischer lymphatischer Leukämie (CLL) in Kombination mit Fludarabin und Cyclophosphamid (R-FC). Patienten mit Vortherapie von Fludarabin sollten über eine Dauer von mindestens 6 Monaten angesprochen haben.

Rheumatoide Arthritis

MabThera in Kombination mit Methotrexat (MTX) ist zur Behandlung erwachsener Patienten mit mittelschwerer bis schwerer aktiver rheumatoider Arthritis (RA) indiziert nach Versagen einer oder mehrerer Therapien mit Tumornekrosefaktor- (TNF-) Hemmern.

ANCA-assoziierte Vaskulitis (AAV)

MabThera in Kombination mit Kortikosteroiden ist zur Behandlung von Patienten mit schwerer aktiver ANCA-assoziierter Vaskulitis (Granulomatose mit Polyangiitis (auch bekannt als Morbus Wegener) und mikroskopische Polyangiitis) indiziert.

Rucaparib  Rubraca® (USA, EU)

According to the NCI website, rucaparib camsylate is an inhibitor of the nuclear enzyme polyadenosine 5’-diphosphoribose (poly-ADP-ribose) polymerase (PARP), with chemosensitizing, radiosensitizing, and antineoplastic activities. Upon administration, rucaparib selectively binds to PARP-1, PARP-2 and PARP-3, and inhibits PARP1-mediated repair of single-strand DNA (ssDNA) breaks via the base-excision repair pathway; this enhances the accumulation of DNA strand breaks and promotes genomic instability and apoptosis. Rucaparib may potentiate the cytotoxicity of DNA-damaging agents and reverse tumor cell resistance to chemotherapy and radiation therapy. Rubraca ist indiziert als Erhaltungstherapie bei erwachsenen Patientinnen mit fortgeschrittenem, platinsensitivem, rezidiviertem, highgrade serösem Ovarial-, Eileiter- oder primärem Peritonealkarzinom, im Anschluss an eine platinbasierte Chemotherapie bei Vorliegen einer vollständigen oder partiellen Remission.

Ruxolitinib - JAKAVI®

Myelofibrose (MF)

  • Zur Behandlung der Splenomegalie oder krankheitsassoziierter Symptome bei Patienten mit Myelofibrose intermediären oder hohen Risikos, mit primärer Myelofibrose oder als Komplikationen einer Polycythaemia vera oder Essentiellen Thrombozythämie.

Polycythaemia vera (PV)

  • Zur Behandlung von Patienten mit Polycythaemia vera, die gegenüber einer Behandlung mit Hydroxyurea oder einer anderen zytoreduktiven Erstlinientherapie resistent sind oder diese nicht vertragen.
