Ibritumomab Tiuxetan

Zevalin®; According to the NCI ibritumomab tiuxetan is an immunoconjugate of the monoclonal antibody ibritumomab conjugated with the linker-chelator tiuxetan, a high affinity, conformationally restricted chelation site for radioisotopes. When bound to indium In 111 or yttrium Y 90, ibritumomab tiuxetan, targeting the CD20 antigen on B cell surfaces, specifically delivers a potentially cytotoxic dose of radiation to B lymphocytes. Ibritumomab is a murine IgG1 kappa monoclonal antibody directed against the CD20 antigen, which is found on the surface of normal and malignant B lymphocytes.

Indikationen/Anwendungsmöglichkeiten gemäss Fachinformation (Stand 2013):

  • Zevalin ist indiziert als Konsolidierungstherapie nach Remissionsinduktion bei vormals unbehandelten Patienten mit follikulärem Lymphom Stadium III oder IV. Der therapeutische Nutzen nach einer Behandlung mit Rituximab in Kombination mit einer Chemotherapie ist noch nicht ausreichend ermittelt.
  • Zevalin ist indiziert für die Behandlung von rezidivierendem oder refraktärem indolenten, follikulären, oder transformierten B-Zell Non-Hodgkin-Lymphom, inklusive auf Rituximab refraktäre follikuläre Non-Hodgkin-Lymphome.
 
 
  • Die Bestimmung der Wirksamkeit der Behandlung mit Zevalin in der rezidivierenden oder refraktären Patientengruppe basiert auf der Gesamtansprechrate. Die Auswirkungen auf das Überleben sind nicht bekannt.

Monoclonal Antibodies

Ibrutinib - IMBRUVICA®
Ibrutinib - IMBRUVICA®  ist ein oral bioverfügbarer niedermolekularer Inhibitor der Bruton-Tyrosinkinase (BTK) mit einer potentiell antineoplastischen Aktivität. Bei oraler Verabreichung bindet Ibrutinib an die BTK  und hemmt ihre Aktivität irreversibel, wodurch sowohl die B-Zell-Aktivierung als auch die B-Zell-vermittelte Signalisierung verhindert wird. Dies führt zu einer Hemmung des Wachstums von malignen B-Zellen, die BTK überexprimieren.
BTK, ein Mitglied der src-bezogenen BTK/Tec-Familie von zytoplasmatischen Tyrosinkinasen ist für die B-Zell-Rezeptor-Signalisierung erforderlich und spielt eine Schlüsselrolle bei der B-Zell-Reifung. BTK ist in einer Anzahl von B-Zell-Malignitäten überexprimiert. Die Expression von BTK in Tumorzellen ist auch mit einer erhöhten Proliferation und dem Überleben assoziiert.

 

Indikationen/Anwendungsmöglichkeiten gemäss Compendium®:
Mantelzelllymphom (MCL)
  • Behandlung von erwachsenen Patienten mit Mantelzelllymphom, charakterisiert durch Translokation t(11;14) und/oder Expression von Cyclin D1, in denen kein partielles Ansprechen erreicht wurde mit vorheriger Therapie oder die eine Progression nach der vorherigen Therapie gezeigt haben.
Chronisch lymphatische Leukämie (CLL)
  • Behandlung erwachsener Patienten mit chronisch lymphatischer Leukämie (CLL):

    • die nicht vorbehandelt sind und für die eine Fludarabin-basierte Immunchemotherapie in voller Dosis nicht in Frage kommt oder
    • die mindestens eine vorangehende Therapie erhalten haben oder
    • die eine 17p-Deletion oder eine TP53‑Mutation aufweisen
Morbus Waldenström (MW)
  • Behandlung erwachsener Patienten mit Morbus Waldenström (MW), die mindestens eine vorangehende Therapie erhalten haben oder zur Erstlinien-Therapie bei Patienten, welche für eine Chemo-Immuntherapie nicht in Frage kommen und für Erwachsene mit symptomatischem MW in Kombination mit Rituximab ab der ersten Linie.

Bruton's Tyrosine Kinase

Icotinib

According to the NCI website, icotinib hydrochloride is an orally available quinazoline-based inhibitor of epidermal growth factor receptor (EGFR), with potential antineoplastic activity. Icotinib selectively inhibits the wild-type and several mutated forms of EGFR tyrosine kinase. This may lead to an inhibition of EGFR-mediated signal transduction and may inhibit cancer cell proliferation. EGFR, a receptor tyrosine kinase, is upregulated in a variety of cancer cell types.

Idarubicin - ZAVEDOS®

According to the NCI idarubicin is a semisynthetic 4-demethoxy analogue of the antineoplastic anthracycline antibiotic daunorubicin. Idarubicin intercalates into DNA and interferes with the activity of topoisomerase II, thereby inhibiting DNA replication, RNA transcription and protein synthesis. Due to its high lipophilicity, idarubicin penetrates cell membranes more efficiently than other anthracycline antibiotic compounds.

Indikationen/Anwendungsmöglichkeiten gemäss Compendium®:

Zavedos Solution (intravenöse Verabreichung)

  • Akute myeloische Leukämie in Kombination mit anderen Zytostatika.
  • Akute lymphatische Leukämie als Reserve-Arzneimittel in Kombination mit anderen Zytostatika.

Zavedos, Kapseln

  • Remissionsinduktion bei akuter myeloischer Leukämie als Teil von attenuierten Kombinationsschemata in der Behandlung älterer Patienten. Die Kapseln sollen nur angewendet werden, wenn die intravenöse Gabe aus medizinischen (z.B. schlechte Venenverhältnisse) oder psychologischen Gründen (z.B. Verweigerung der i.v.-Therapie) nicht möglich ist. Sie sollen nicht für die palliative Therapie eingesetzt werden.
  • Behandlung des fortgeschrittenen Mammakarzinoms nach Versagen der «first line» Chemotherapie ohne Anthrazykline bei Patienten, bei denen eine Anthrazyklintherapie indiziert ist, jedoch die intravenöse Gabe wegen medizinischer, psychologischer oder sozialer Gründe nicht möglich ist.

Anthrazykline

Idasanutlin

According to the NCI website idasanutlin is an orally available, small molecule, antagonist of MDM2 (mouse double minute 2; Mdm2 p53 binding protein homolog), with potential antineoplastic activity. Idasanutlin binds to MDM2 blocking the interaction between the MDM2 protein and the transcriptional activation domain of the tumor suppressor protein p53. By preventing the MDM2-p53 interaction, p53 is not enzymatically degraded and the transcriptional activity of p53 is restored. This may lead to p53-mediated induction of tumor cell apoptosis. MDM2, a zinc finger nuclear phosphoprotein and negative regulator of the p53 pathway, is often overexpressed in cancer cells and has been implicated in cancer cell proliferation and survival.

Idecabtagene Vicleucel

According to the NCI website, Idecabtagene Vicleucel is a preparation of autologous peripheral blood T-lymphocytes (PBTLs) that have been genetically modified to express a chimeric antigen receptor (CAR) specific for the B-cell maturation antigen (BCMA), with potential immunostimulating and antineoplastic activities. Upon administration, idecabtagene vicleucel specifically recognizes and kills BCMA-expressing tumor cells. BCMA, a tumor-specific antigen and a receptor for both a proliferation-inducing ligand (APRIL) and B-cell activating factor (BAFF), is a member of the tumor necrosis factor receptor superfamily (TNFRSF) and plays a key role in plasma survival; it is found on the surfaces of plasma cells and overexpressed on malignant plasma cells.

View Trials Investigating Idecabtagene vicleucel (ide-cel; bb2121) in Multiple Myeloma

Idelalisib - ZYDELIG®

Idelalisib - Zydelig® ist ein oral bioverfügbarer, niedermolekularer Inhibitor der Delta-Isoform der 110 kDa katalytischen Untereinheit der Klasse-I-Phosphoinositid-3-Kinase (PI3K) mit potentiell immunmodulierenden und antineoplastischen Aktivitäten. Idelalisib hemmt die Produktion des zweiten Messengers Phosphatidylinositol-3,4,5-Triphosphat (PIP3), was die Aktivierung des PI3K Signalweges und die Proliferation, Motilität und das Überleben von Tumorzellen verhindert. Im Gegensatz zu anderen Isoformen von PI3K, wird PI3K-Delta in erster Linie in hämatopoetischen Zelllinien exprimiert. Die gezielte Hemmung von PI3K-delta wurde dazu entwickelt, die PI3K Signalisierung in normalen, nicht-neoplastischen Zellen zu bewahren. 

Indikationen/Anwendungsmöglichkeiten gemäss Arzneimittel-Kompendium der Schweiz®:

In Kombination mit Rituximab zur Behandlung von Patienten mit chronischer lymphatischer B‑Zell Leukämie (B‑CLL) und Komorbidität:

  • die mindestens eine Vortherapie erhalten haben, oder
  • bei Hochrisiko-CLL‑Patienten (17p‑Deletion, TP53‑Mutation), die für eine Chemoimmuntherapie ungeeignet sind.

Als Monotherapie zur Behandlung von Patienten mit rezidivierendem follikulärem Lymphom (FL), die zwei vorausgegangene Therapielinien erhalten haben.

Ifosfamide - HOLOXAN®

Holoxan®; According to the NCI ifosfamide A synthetic analogue of the nitrogen mustard cyclophosphamide with antineoplastic activity. Ifosfamide alkylates and forms DNA crosslinks, thereby preventing DNA strand separation and DNA replication. This agent is a prodrug that must be activated through hydroxylation by hepatic microsomal enzymes.

Alkylating Agents

Imatinib - GLIVEC®/GLIVEC GIST sowie verschiedene Generika

Imatinib ist ein Tyrosinkinase-Inhibitor mit antineoplastischer Aktivität. Imatinib bindet an eine intrazelluläre Tasche, die innerhalb der Tyrosinkinasen (TK) angeordnet ist. Dadurch werden die ATP Bindung und die Phosphorylierung sowie die anschließende Aktivierung von Wachstumsrezeptoren und deren nachgeschalteten Signaltransduktionswege verhindert. Dieses Mittel hemmt die durch das BCR-ABL-Onkogen codierte TK sowie die durch die c-kit und Plättchenwachstumsfaktor-Rezeptor (PDGFR) Onkogene kodierten Rezeptor TKs. Die Hemmung der BCR-ABL-TK führt zu einer verringerten Proliferation und verstärkte Apoptose in malignen Zellen von Philadelphia-positiven (Ph +) hämatologischen Malignitäten wie CML und ALL; Auswirkungen auf die c-kit TK-Aktivität hemmen die Mastzellen und die Zellproliferation in Krankheiten, die  c-kit überexprimieren, wie beispielsweise bei der Mastozytose und den gastrointestinalen Stromatumoren (GIST).

Indikationen/Anwendungsmöglichkeiten für Glivec® (vgl. Fachinformation Compendium®):

    • Behandlung von erwachsenen und pädiatrischen Patienten mit Ph+ chronisch-myeloischer Leukämie (Ph+ CML) in der chronischen Phase, in der akzelerierten Phase sowie in der Blastenkrise.
    • Behandlung von Erwachsenen und pädiatrischen Patienten mit Ph+ akuter lymphatischer Leukämie (Ph+ ALL) in Kombination mit Standardchemotherapie.
    • Behandlung von erwachsenen Patienten mit Hypereosinophilensyndrom (HES), atypischen myelodysplastischen/myeloproliferativen Erkrankungen (MDS/MPD) oder aggressiver systemischer Mastozytose (SM) einhergehend mit Eosinophilie und einer platelet-derived growth factor (PDGF)-Rezeptor-alpha oder -beta Mutation oder einem FIP1L1-PDGFR-alpha Fusionsprotein.
    • Behandlung von Erwachsenen mit unresezierbarem, rezidivierendem oder metastasiertem Dermatofibrosarkom protuberans (DFSP).

Indikationen/Anwendungsmöglichkeiten gemäss Compendium® beGlivec GIST (vgl. Fachinformation Compendium®)

  • Behandlung unresezierbarer und/oder metastasierter maligner gastrointestinaler Stromatumoren (GIST) bei Erwachsenen.
  • Adjuvante Behandlung Erwachsener mit relevantem Risiko eines Rezidivs nach Resektion KIT-(CD117)-positiver GIST: Patienten mit einem niedrigen Rezidivrisiko sollen keine adjuvante Behandlung erhalten.

Infigratinib

According to the NCI website, Infigratinib is an orally bioavailable pan inhibitor of human fibroblast growth factor receptors (FGFRs) with potential antiangiogenic and antineoplastic activities. Infigratinib selectively binds to and inhibits the activities of FGFRs, which may result in the inhibition of tumor angiogenesis and tumor cell proliferation, and the induction of tumor cell death. FGFRs are a family of receptor tyrosine kinases which may be upregulated in various tumor cell types and may be involved in tumor cell differentiation and proliferation, tumor angiogenesis, and tumor cell survival. Iniparib

According to the NCI website Iniparib is a small-molecule prodrug inhibitor of the nuclear enzyme poly(ADP-ribose) polymerase-1 (PARP-1) with potential chemosensitizing, radiosensitizing and antineoplastic activities. In vivo, iniparib (PARP-1 inhibitor BSI-201) is converted to the active drug, which selectively binds to PARP-1 and inhibits PARP-1- mediated DNA repair. Consequently, this agent may enhance the cytotoxicity of DNA-damaging agents and reverse tumor cell resistance to chemotherapy and radiation therapy. In addition, iniparib may exhibit direct antineoplastic activity against cancers defective in DNA repair. PARP-1 catalyzes post-translational ADP-ribosylation of nuclear proteins and is activated by single-strand DNA (ssDNA) breaks.

The role of PARP in regulating the cellular processes 

According to the website of SELLECKBIO PARP, one of the most abundant proteins in the nucleus, has the principal role in regulating the cellular processes that are most relevant in terms of pathophysiology and experimental therapy. PARP1 which is a 116-kDa protein consists of three main domains is the most important member of the PARP family and can be a DNA-damage sensor and signalling molecule because the ZINC FINGERS of PARP recognize singleand double-stranded DNA breaks. PARP can be cleaved into inactive fragments by caspases or matrix metalloproteinases (MMPs). The phosphorylation of PARP by protein kinase-C (PKC), through the inhibition of its DNA binding, also suppresses PARP activity. PARP, through physical interactions and/or catalytic actions, promotes activator protein-1 (AP1) and nuclear factor-κB (NF-κB)-dependent signal-transduction processes and can suppress the activation of the cytoprotective AKT pathway; therefore, the inhibition of PARP can suppress AP1 and NF-κB activation and induce AKT phosphorylation.

Inotuzumab ozogamicin - BESPONSA®

According to the NCI website inotuzumab ozogamicin is a CD22-targeted cytotoxic immunoconjugate composed of a humanized IgG4 anti-CD22 antibody covalently linked to N-acetyl-gamma-calicheamicin dimethyl hydrazide (CalichDMH) with potential antineoplastic activity. Inotuzumab ozogamicin is rapidly internalized upon binding of the antibody moiety to B cell-specific CD22 receptors, delivering the conjugated CalichDMH intracellularly; the CalichDMH moiety binds to the minor groove of DNA in a sequence-specific manner, resulting in double-strand DNA breaks and apoptosis.

 

Indikationen/Anwendungsmöglichkeiten gemäss Compendium®
  • Besponsa wird eingesetzt zur Behandlung von erwachsenen Patienten mit CD22-positiver, rezidivierender oder refraktärer B-Vorläuferzellen-ALL (akute lymphoblastische Leukämie). Erwachsene Patienten mit Philadelphia Chromosom positiver (Ph+) rezidivierender oder refraktärer B-Vorläuferzellen-ALL, sollten Therapieversagen bei einer Vorbehandlung mit mindestens einem Tyrosin Kinase Inhibitor (TKI) gezeigt haben.
 
Interferon Alfa - ROFERON®-A

According to the NCI website Interferon Alfa-2a (Intron A®, Roferon®-A) is a drug used to treat some infections caused by viruses and several types of cancer. These include hairy cell leukemia, melanoma, and follicular lymphoma. It is a form of interferon alfa (a substance normally made by cells of the immune system) that is made in the laboratory. It is a type of biological response modifier. Also called IFN alpha-2B, Intron A, and recombinant interferon alfa-2b.

Indikationen/Anwendungsmöglichkeiten gemäss Compendium® (z.B. Roferon®):

  • Haarzellen-Leukämie.
  • Kutanes T-Zell-Lymphom (Mycosis fungoides und Sézary-Syndrom).
  • Chronisch-myeloische Leukämie im chronischen Stadium.
  • Kaposi-Sarkom bei AIDS-Patienten.
  • Adjuvante Therapie des operativ entfernten malignen Melanoms Stadium II nach AJCC (Dicke des Tumors nach Breslow >1,5 mm; keine Lymphknoten- oder Fernmetastasen).
  • Nierenzellkarzinom im fortgeschrittenen Stadium in Kombination mit Vinblastin.
  • Erstlinientherapie von nephrektomierten Patienten mit fortgeschrittenem und/oder metastasierendem Nierenzellkarzinom in Kombination mit Bevacizumab.

Chronische Hepatitis C

Chronische, histologisch nachgewiesene Hepatitis C bei erwachsenen Patienten, bei denen HCV-Antikörper, HCV-RNA und erhöhte Serumspiegel der Aminotransferase (ALT) ohne Leberdekompensation vorliegen.

Die Wirksamkeit von Interferon alfa-2a bei der Behandlung von Rückfall-Patienten mit chronischer Hepatits C wird durch die Kombination mit Ribavirin erhöht.

Für die Kombinationsbehandlung ist die Arzneimittelinformation für Ribavirin zu beachten.

Für nicht vorbehandelte (Interferon-naive) Patienten liegt für die Kombination mit Ribavirin nur eine kleine offene Vergleichsstudie vor.

Chronische Hepatitis B

Roferon ist indiziert zur Erhöhung der Wahrscheinlichkeit einer Serokonversion bei Kaukasiern mit chronisch aktiver Hepatitis B ohne Leberdekompensation. Die chronisch aktive Hepatitis B muss durch eine Leberbiopsie, durch eine Transaminasenerhöhung (>6 Monate) und durch Marker der Virusreplikation (HBV-DNA und HbeAg positiv) dokumentiert sein. Ein positiver Einfluss auf die Ausbildung einer Leberzirrhose oder eines hepatozellulären Karzinoms konnte nicht gezeigt werden.

Ipatasertib
According to the NCI website Ipatasertid is an orally bioavailable inhibitor of the serine/threonine protein kinase Akt (protein kinase B) with potential antineoplastic activity. Ipatasertib binds to and inhibits the activity of Akt in a non-ATP-competitive manner, which may result in the inhibition of the PI3K/Akt signaling pathway and tumor cell proliferation and the induction of tumor cell apoptosis. Activation of the PI3K/Akt signaling pathway is frequently associated with tumorigenesis and dysregulated PI3K/Akt signaling may contribute to tumor resistance to a variety of antineoplastic agents.
 
Ipilimumab - YERVOY®

Yervoy® - A monoclonal antibody directed against cytotoxic T-lymphocyte-associated antigen-4 (CTLA4), an antigen that is expressed on activated T-cells and exhibits affinity for B7 co-stimulatory molecules.

Indikationen/Anwendungsmöglichkeiten gemäss Compendium®:

Melanom
  • YERVOY ist für die Behandlung von fortgeschrittenen (nicht resezierbaren oder metastasierten) Melanomen bei Erwachsenen indiziert.
Nierenzellkarzinom (RCC)
  • YERVOY ist in Kombination mit Nivolumab für die Behandlung des fortgeschrittenen (nicht resezierbaren oder metastasierten) Nierenzellkarzinoms bei vorher unbehandelten erwachsenen Patienten mit intermediärem/ungünstigem Risikoprofil indiziert.
Kolorektales Karzinom (CRC)

Irinotecan - CAMPTO® sowie Generika

According to the NCI website, Irinotecan, a prodrug, is converted to a biologically active metabolite 7-ethyl-10-hydroxy-camptothecin (SN-38) by a carboxylesterase-converting enzyme. One thousand-fold more potent than its parent compound irinotecan, SN-38 inhibits topoisomerase I activity by stabilizing the cleavable complex between topoisomerase I and DNA, resulting in DNA breaks that inhibit DNA replication and trigger apoptotic cell death.

Indikationen/Anwendungsmöglichkeiten gemäss Compendium®:

  • Campto ist in Kombination mit 5-FU/Folinsäure oder in Kombination mit Bevacizumab und 5-FU/Folinsäure zur Erstlinientherapie bei Patienten mit metastasierendem Karzinom des Kolons oder Rektums indiziert.
  • Campto ist als Monotherapie indiziert zur Behandlung des fortgeschrittenen Kolorektalkarzinoms nach Therapieversagen mit 5-Fluorouracil (Second-line Therapie).
  • Campto ist in Kombination mit Cetuximab indiziert zur Behandlung von Patienten mit EGFR (epidermal growth factor receptor)-exprimierendem metastasierendem Kolorektalkarzinom, wenn eine zytotoxische Therapie unter Einschluss von Campto versagt hat.

Isatuximab SARCLISA® (EU)

According to the NCI website the anti-CD38 monoclonal antibody SAR650984 or Isatuximab is a humanized IgG1 monoclonal antibody directed against the cell surface glycoprotein CD-38 with potential antineoplastic activity. Anti-CD38 monoclonal antibody SAR650984 specifically binds to CD38 on CD38-positive tumor cells. This may trigger antitumoral antibody-dependent cellular cytotoxicity (ADCC), complement-dependent cytotoxicity (CDC) and apoptosis eventually leading to cell lysis in CD38-expressing tumor cells. CD38, a type II transmembrane glycoprotein, is present on various immune cells and hematologic malignancies, and its expression has been correlated with poor prognosis.

 

Full indication according to the EMA Committee for Medicinal Products for Human Use (CHMP):

“Sarclisa is indicated, in combination with pomalidomide and dexamethasone, for the treatment of adult patients with relapsed and refractory multiple myeloma (MM) who have received at least two prior therapies including lenalidomide and a proteasome inhibitor (PI) and have demonstrated disease progression on the last therapy.”

Ivosidenib – TIBSOVO® (USA)

According to the NCI website ivosidenib is an orally available inhibitor of isocitrate dehydrogenase type 1 (IDH1), with potential antineoplastic activity. Upon administration, ivosidenib specifically inhibits a mutated form of IDH1 in the cytoplasm, which inhibits the formation of the oncometabolite, 2-hydroxyglutarate (2HG). This may lead to both an induction of cellular differentiation and an inhibition of cellular proliferation in IDH1-expressing tumor cells. IDH1, an enzyme in the citric acid cycle, is mutated in a variety of cancers; it initiates and drives cancer growth by both blocking cell differentiation and catalyzing the formation of 2HG. Check for active clinical trials using this agent.

Ixazomib - NINLARO®

According to the NCI website ixazomib citrate is an orally bioavailable second generation proteasome inhibitor (PI) with potential antineoplastic activity. Ixazomib inhibits the activity of the proteasome, blocking the targeted proteolysis normally performed by the proteasome, which results in an accumulation of unwanted or misfolded proteins; disruption of various cell signaling pathways may follow, resulting in the induction of apoptosis. Compared to first generation PIs, second generation PIs may have an improved pharmacokinetic profile with increased potency and less toxicity. Proteasomes are large protease complexes that degrade unneeded or damaged proteins that have been ubiquinated. Check for active clinical trials using this agent. 

Indikationen/Anwendungsmöglichkeiten gemäss Compendium®

NINLARO in Kombination mit Lenalidomid und Dexamethason ist für die Behandlung von erwachsenen Patienten mit multiplem Myelom indiziert,

  • die mindestens eine vorangegangene Therapie erhalten haben und Hochrisikomerkmale aufweisen, oder
  • die mindestens zwei vorangegangene Therapien erhalten haben.
