Elotuzumab - EMPLICITI®

According to the NCI website Elotuzumab is a humanized monoclonal antibody directed against the human CS1 (CD2 subset 1, CRACC, SLAMF7) antigen with potential antineoplastic activity. Elotuzumab binds to the CS1 antigen, which may trigger antibody-dependent cellular cytotoxicity (ADCC) in cells expressing CS1. CS1 is a cell surface glycoprotein belonging to the CD2 subset of the immunoglobulin superfamily (IgSF) and is highly expressed by multiple myeloma cells, but minimally expressed by normal cells.

 

Indikationen/Anwendungsmöglichkeiten gemäss Compendium®

  • Empliciti ist in Kombination mit Lenalidomid und Dexamethason für die Behandlung des multiplen Myeloms bei erwachsenen Patienten indiziert, die zuvor eine oder mehrere Therapien erhalten haben und die Progredienz oder eine Unverträglichkeit zur letzten Therapie gezeigt haben.
  • Empliciti ist in Kombination mit Pomalidomid und Dexamethason für die Behandlung des multiplen Myeloms bei erwachsenen Patienten indiziert, die zuvor mindestens zwei Therapien inklusive Lenalidomid und einen Proteasom-Inhibitor erhalten haben und die Progredienz zur letzten Therapie gezeigt haben.

Emactuzumab

According to the NCI website Emactuzumab is a humanized monoclonal antibody directed against the tyrosine kinase receptor colony stimulating factor 1 receptor (CSF1R; CSF-1R; CD115), also known as macrophage colony-stimulating factor receptor (M-CSFR), with potential antineoplastic and immunomodulating activities. Upon administration, emactuzumab binds to CSF1R expressed on macrophages and inhibits the binding of colony-stimulating factor-1 (CSF-1) to CSF1R. This prevents CSF1R activation and CSF1R-mediated signaling in these cells, which blocks the production of inflammatory mediators by macrophages and reduces inflammation. By blocking both the activity of CSF1R-dependent tumor-associated macrophages (TAMs) and the recruitment of TAMs to the tumor microenvironment, emactuzumab enhances T-cell infiltration and antitumor T-cell immune responses, which inhibits the proliferation of tumor cells. TAMs play key roles in immune suppression and promoting inflammation, tumor cell proliferation and survival. Check for active clinical trials using this agent.

Enasidenib – IDHIVA (USA)

According to the NCI website enasidenib mesylate  is the the mesylate salt form of enasidenib, an orally available inhibitor of specific mutant forms of the mitochondrial enzyme isocitrate dehydrogenase type 2 (IDH2), with potential antineoplastic activity. Upon administration, enasidenib specifically inhibits various mutant forms of IDH2, including the IDH2 variants R140Q, R172S, and R172K, which inhibits the formation of 2-hydroxyglutarate (2HG). This may lead to both an induction of cellular differentiation and an inhibition of cellular proliferation in IDH2-expressing tumor cells. IDH2, an enzyme in the citric acid cycle, is mutated in a variety of cancers; it initiates and drives cancer growth by blocking differentiation and the production of the oncometabolite 2HG. Celgene Europe B.V. withdrew its application for a marketing authorisation of Idhifa for the treatment of adults with acute myeloid leukaemia (AML), a cancer of white blood cells.

Encorafenib - BRATOVI®

According to the NCI website, encorafenib is an orally available Raf kinase inhibitor with potential antineoplastic activity. Encorafenib specifically inhibits Raf kinase, a serine/threonine enzyme in the RAF/mitogen-activated protein kinase kinase (MEK)/extracellular signal-related kinase (ERK) signaling pathway. By inhibiting the activation of the RAF/MEK/ERK signaling pathway, the administration of encorafenib may result in a decrease in proliferation of tumor cells. The Raf mutation BRAF V600E is frequently upregulated in a variety of human tumors and results in the constitutive activation of the RAF/MEK/ERK signaling pathway that regulates cellular proliferation and survival. Ensartinib

According to the NCI website, Ensartinib is an orally available small molecule inhibitor of the receptor tyrosine kinase anaplastic lymphoma kinase (ALK) with potential antineoplastic activity. Upon oral administration, ensartinib binds to and inhibits ALK kinase, ALK fusion proteins and ALK point mutation variants. Inhibition of ALK leads to the disruption of ALK-mediated signaling and eventually inhibits tumor cell growth in ALK-expressing tumor cells. ALK belongs to the insulin receptor superfamily and plays an important role in nervous system development. ALK is not expressed in healthy adult human tissue but ALK dysregulation and gene rearrangements are associated with a series of tumors; ALK mutations are associated with acquired resistance to small molecule tyrosine kinase inhibitors. Entrectinib – ROZLYTREK® (USA)

According to the NCI website, Entrectinib is an orally bioavailable inhibitor of the tyrosine kinases tropomyosin receptor kinases (Trk) A, B and C, C-ros oncogene 1 (ROS1) and anaplastic lymphoma kinase (ALK), with potential antineoplastic activity. Upon administration, entrectinib binds to and inhibits TrkA, TrkB, TrkC, ROS1 and ALK. Inhibition of these kinases may result in a disruption of TrkA-, TrkB-, TrkC-, ROS1-, and ALK-mediated signaling. This leads to an induction of apoptosis and an inhibition of tumor cell proliferation in tumor cells that express these kinases. Enzalutamid - XTANDI®
Enzalutamid - Xtandi® ist ein oral bioverfügbarer, organischer, nicht-steroidaler, kleinmolekularer Androgenrezeptor (AR) mit einer potenziellen antineoplastischen Aktivität. Durch einen Mechanismus, der sich von dem bei anderen zugelassenen AR-Antagonisten unterscheidet, hemmt Enzalutamid die Aktivität der AR in der Prostatakrebszelle. Dies führt zu einer Verringerung der Prostatakrebs-Zellproliferation und dementsprechend zu einer Verringerung der Serum-Prostata-spezifischen Antigens (PSA). Die AR-Überexpression beim Prostata-Karzinom ist ein wichtiger Mechanismus beim hormonresistenten Prostata-Karzinom.
 

Indikationen gemäss Compendium.ch®

  • In Kombination mit LHRH-Agonisten zur Behandlung von Männern mit metastasiertem kastrationsresistentem Prostatakarzinom (CRPC) mit asymptomatischem oder mild symptomatischem Verlauf nach Versagen der Androgenentzugstherapie, und bei denen eine Chemotherapie noch nicht klinisch indiziert ist.
  • In Kombination mit LHRH Agonisten zur Behandlung von Männern mit metastasierendem CRPC bei Progression unter oder nach Docetaxel Therapie.

Anthrazykline

Epoetin alpha - BINOCRIT®, EPREX®/ Epoetin beta - RECORMON®/ Epoetin theta - EpoTheta Teva

According to the NCI epoetin alfa is a recombinant therapeutic agent which is chemically identical to or similar to the endogenous cytokine human erythropoietin (EPO). Produced primarily by cells of the peritubular capillary endothelium of the kidney in response to hypoxia, circulating EPO binds to EPO receptors on the surface of committed erythroid progenitors in the bone marrow resulting in their replication and maturation into functional erythrocytes.

Indikationen/Anwendungsmöglichkeiten gemäss Compendium®:

Epoetin alfa: Binocrit®, Eprex®
Anämie bei chronischer Niereninsuffizienz
Symptomatische, bluttransfusionsbedürftige, chronische Anämie bei chronischer, fortgeschrittener Niereninsuffizienz (Prädialyse- und Dialysepatienten).
Tumorpatienten
Behandlung der symptomatischen Anämie und Reduktion des Transfusionsbedarfs bei erwachsenen Tumorpatienten mit einem Hämoglobinwert von <10,5 g/dl, bei welchen eine Chemotherapie über eine Mindestdauer von 2 Monaten vorgesehen ist.
Bei nicht platinhaltiger Chemotherapie wurde in den klinischen Studien die Behandlung mit Eprex im Allgemeinen nach 2–3 Zyklen Chemotherapie begonnen (siehe «Eigenschaften/Wirkungen»).
Präoperativ zur Vermeidung von Fremdbluttransfusionen
Stimulierung der Erythropoese vor einem grossen orthopädischen Eingriff zur Reduktion von allogenen Bluttransfusionen und zur Korrektur einer postoperativen Anämie bei Erwachsenen ohne Eisenmangel. Die Behandlung mit Eprex sollte nur bei Patienten mit mittelschwerer Anämie (Hb 10–13 g/dl) und einem erwarteten Blutverlust von 900–1’800 ml durchgeführt werden.
Präoperativ mit Eigenblutspende
Stimulierung der Erythropoese zwecks Erhöhung der Eigenblutspenden und Beschränkung des Hämoglobinabfalls bei Patienten, die für grössere Wahloperationen vorgesehen sind und die erwartungsgemäss nicht die erforderliche Anzahl Einheiten Eigenblut spenden können.
Die Behandlung mit Eprex sollte nur bei Patienten ohne Eisenmangel mit mittelschwerer Anämie (Hb 10–13 g/dl) durchgeführt werden, falls blutgewinnende Massnahmen nicht verfügbar oder unzureichend sind, und der geplante operative Eingriff einen grossen Blutvolumenersatz fordert (4 oder mehr Einheiten Blut bei Frauen; 5 oder mehr Einheiten Blut bei Männern).
Fremdblutsparende Massnahmen sollten immer bei operativen Eingriffen zur Anwendung kommen.
Anwendungsbeispiele: Niedrige Hämoglobinwerte, Eigenblutbedarf ≥5 Einheiten, kurze präoperative Vorbereitungszeit.


Epoetin beta: Recormon® PS (Für genauere Angaben zu Mircera® vgl. Kompendium)
Anämie bei Patienten mit chronischer Nierenerkrankung
Behandlung der symptomatischen, transfusionsbedürftigen, renalen Anämie bei chronischer Nierenerkrankung bei dialysierten und nicht dialysierten erwachsenen Patienten und Kindern über 2 Jahre (für Hb-Grenzwerte, siehe «Dosierung/Anwendung»).
Tumorpatienten
Behandlung der symptomatischen Anämie bei erwachsenen Patienten mit nicht-myeloiden malignen Erkrankungen, die eine Chemotherapie erhalten (für Hb-Grenzwerte, siehe «Dosierung/Anwendung»).
Gewinnung von Eigenblut (Eigenblutspendeprogramm)
Steigerung der Menge an Eigenblut bei Patienten, die in ein Eigenblutspendeprogramm zur Vermeidung von Fremdbluttransfusionen einbezogen sind. Die Anwendung in dieser Indikation muss gegenüber dem berichteten erhöhten Risiko für thromboembolische Ereignisse abgewogen werden. Die Behandlung ist bei Patienten mit mässiggradiger Anämie (Hb 10–13 g/dl (6,21–8,07 mmol/l), kein Eisenmangel) indiziert, falls blutsparende Verfahren nicht verfügbar oder unzureichend sind, sofern der geplante grössere chirurgische Eingriff grosse Mengen an Blut erfordert (4 oder mehr Blutkonserven bei Frauen bzw. 5 oder mehr bei Männern) oder die Zeitspanne zu kurz ist, um die benötigte Eigenblutmenge zu gewinnen. 

Epoetin theta: EpoTheta-Teva®
Behandlung einer symptomatischen Anämie infolge chronischer Niereninsuffizienz bei erwachsenen Patienten
Behandlung einer symptomatischen Anämie bei erwachsenen Krebspatienten mit nicht-myeloischen malignen Erkrankungen, die eine Chemotherapie erhalten.

Eribulin - HALAVEN®

According to the NCI eribulin is the mesylate salt of a synthetic analogue of halichondrin B, a substance derived from a marine sponge (Lissodendoryx sp.) with antineoplastic activity. Eribulin binds to the vinca domain of tubulin and inhibits the polymerization of tubulin and the assembly of microtubules, resulting in inhibition of mitotic spindle assembly, induction of cell cycle arrest at G2/M phase, and, potentially, tumor regression.

Indikationen/Anwendungsmöglichkeiten gemäss Arzneimittel-Kompendium der Schweiz®:

HALAVEN ist indiziert

  • als Monotherapie für die Behandlung von lokal fortgeschrittenem oder metastasierendem HER2 negativem Mammakarzinom nach Versagen einer Chemotherapie im fortgeschrittenen Stadium. Patienten sollten zuvor ein Anthrazyklin und ein Taxan erhalten haben, es sei denn diese Behandlung war ungeeignet.
  • für die Behandlung des inoperablen Liposarkoms bei Progression nach einer Chemotherapie im fortgeschrittenen oder metastasierenden Stadium bei Erwachsenen. Patienten sollten zwei vorhergehende Chemotherapien erhalten haben, davon sollte eines ein Anthrazyklin sein, es sei denn diese Behandlung ist ungeeignet.

Erlotinib - TARCEVA®

Erlotinib-Hydrochlorid - Tarceva® ist das Hydrochloridsalz eines Chinazolin-Derivats und besitzt antineoplastische Eigenschaften. Im Wettbewerb mit Adenosintriphosphat bindet Erlotinib reversibel an die intrazelluläre katalytische Domäne der Tyrosinkinase des epidermalen Wachstumsfaktorrezeptors (EGFR). Dadurch wird die reversible EGFR-Phosphorylierung gehemmt und die mit der EGFR-Aktivierung assoziierte tumorfördernde Signaltransduktion blockiert. 

Indikationen/Anwendungsmöglichkeiten gemäss Compendium®:

  • Tarceva ist für die Behandlung von Patienten mit lokal fortgeschrittenem oder metastasierendem nicht-kleinzelligem Lungenkrebs indiziert, bei denen mindestens eine vorgängige Chemotherapie wirkungslos geblieben ist.
  • Zur Erhaltungsbehandlung bei Patienten mit lokal fortgeschrittenem oder metastasiertem nicht-kleinzelligem Lungenkrebs im Anschluss an 4 Behandlungszyklen einer platinbasierten First-Linie Standardchemotherapie, wenn die Erkrankung nicht progredient ist und nicht ausreichend auf die Chemotherapie angesprochen hat.
  • Tarceva ist für die Erstlinienbehandlung von Patienten mit lokal fortgeschrittenem oder metastasiertem nicht-kleinzelligem Lungenkarzinom (NSCLC) mit EGFR-aktivierenden Mutationen indiziert.

Zytostatikum

Etoposid - ETOPOPHOS®, VEPESID®, ETOPOSID Sandoz®

Etopophos®, Vepesid®, Etoposid Sandoz® according to NCI website etoposide is a semisynthetic derivative of podophyllotoxin, a substance extracted from the mandrake root Podophyllum peltatum. Possessing potent antineoplastic properties, etoposide binds to and inhibits topoisomerase II and its function in ligating cleaved DNA molecules, resulting in the accumulation of single- or double-strand DNA breaks, the inhibition of DNA replication and transcription, and apoptotic cell death. Etoposide acts primarily in the G2 and S phases of the cell cycle.

Indikationen/Anwendungsmöglichkeiten gemäss Compendium®:

In Kombination mit anderen Zytostatika oder als Monotherapie bei akuter myeloischer Leukämie, Morbus Hodgkin, Nicht-Hodgkin-Lymphomen in fortgeschrittenen Stadien, kleinzelligem Lungenkarzinom, Keimzelltumoren; in Reserveschemata bei einer Reihe anderer Malignome wie Chorionkarzinom, nicht-kleinzelligem Lungenkarzinom.

Topoisomerase II Inhibitor

Everolimus - AFINITOR®

Everolimus - Afinitor® ist ein Derivat des natürlichen makrozyklischen Laktons Sirolimus mit immunsuppressiven und anti-angiogenen Eigenschaften. In Zellen bindet Everolimus an das Immunophilin FK-Bindungsprotein-12 (FKBP-12) und erzeugt damit einen immunsuppressiven Komplex, der die Aktivierung von mTOR (mammalian target of rapamycin) hemmt, eine Schlüssel-Kinase der Signalregulierung. Die Hemmung der mTOR Aktivierung führt zur Hemmung der T-Lymphozyten-Aktivierung und Proliferation, die wiederum mit der Antigen- und Zytokin-Stimulierung (IL-2, IL-4 und IL-15) sowie der Hemmung der Antikörperproduktion verbunden sind.

Afinitor®: Indikationen/Anwendungsmöglichkeiten gemäss Arzneimittel-Kompendium der Schweiz®:
  • In Kombination mit Exemestan zur Behandlung von postmenopausalen Frauen mit fortgeschrittenem, Hormonrezeptor positivem, HER 2 negativem Brustkrebs nach Versagen einer Behandlung mit Letrozol oder Anastrozol.
  • Patienten mit fortgeschrittenen, progredienten, gut oder mässig differenzierten neuroendokrinen Tumoren pankreatischen Ursprungs.
  • Patienten mit fortgeschrittenem Nierenzellkarzinom nach Versagen einer Behandlung mit Sunitinib oder Sorafenib.

Exemestan - AROMASIN®/EXEMESTAN Sandoz®

According to the NCI website exemestane is a synthetic androgen analogue. Exemestane binds irreversibly to and inhibits the enzyme aromatase, thereby blocking the conversion of cholesterol to pregnenolone and the peripheral aromatization of androgenic precursors into estrogens. Indikationen/Anwendungsmöglichkeiten gemäss Compendium®:

  • Adjuvante Behandlung von postmenopausalen Frauen mit invasivem frühem Mammakarzinom, welche Östrogen- oder Progesteronrezeptor-positiv oder von unbekanntem Rezeptorstatus sind, nach mindestens zwei Jahren initialer adjuvanter Tamoxifen-Therapie.
  • Behandlung des fortgeschrittenen Mammakarzinoms bei Frauen mit natürlicher oder künstlich herbeigeführter Postmenopause nach Progression unter Antiöstrogenbehandlung.
  • Aromasin ist auch angezeigt als Hormonbehandlung der dritten Wahl des fortgeschrittenen Mammakarzinoms bei Frauen mit natürlicher oder künstlich herbeigeführter Postmenopause nach Progression unter einer Behandlung mit Antiöstrogenen, nicht-steroidalen Aromatasehemmern oder Gestagenen.
