Regorafenib - STIVARGA®

Regorafenib - Stivarga® ist ein oral bioverfügbares Kleinmolekül mit potentiellen antiangiogenen und antineoplastischen Aktivitäten. Regorafenib bindet an und hemmt die vaskulären endothelialen Wachstumsfaktor-Rezeptoren (VEGFRs) 2 und 3 und die Ret-, Kit-, PDGFR- und Raf-Kinasen, was zur Hemmung der Tumor-Angiogenese und Tumorzellproliferation führen kann. VEGFRs sind Rezeptor-Tyrosin-Kinasen, die eine wichtige Rolle bei der Tumorangiogenese spielen; die Rezeptor-Tyrosin-Kinasen RET, KIT und PDGFR und die Serin/Threonin-spezifischen Raf-Kinasen sind an der Tumorzellsignalisierung beteiligt.

Indikationen/Anwendungsmöglichkeiten gemäss Compendium®:

  • Stivarga ist indiziert zur Behandlung von Patienten mit metastasiertem kolorektalem Karzinom (CRC), die zuvor eine fluoropyrimidin-, oxaliplatin- und irinotecanbasierte Chemotherapie, eine anti-VEGF-Therapie und – bei Vorliegen eines RAS-Wildtyps – eine anti-EGFR-Therapie erhalten haben.
  • Stivarga ist indiziert zur Behandlung von Patienten mit metastasierten oder inoperablen gastrointestinalen Stromatumoren (GIST), die zuvor mit zwei Tyrosinkinase Inhibitoren (Imatinib und Sunitinib) behandelt wurden.
  • Stivarga ist indiziert zur Behandlung von Patienten mit Leberzellkarzinom (HCC), die zuvor mit Sorafenib behandelt wurden.

Ribociclib - KISQALI®

According to the NCI website Ribociclib is an orally available cyclin-dependent kinase (CDK) inhibitor targeting cyclin D1/CDK4 and cyclin D3/CDK6 cell cycle pathway, with potential antineoplastic activity. CDK4/6 inhibitor LEE011 specifically inhibits CDK4 and 6, thereby inhibiting retinoblastoma (Rb) protein phosphorylation. Inhibition of Rb phosphorylation prevents CDK-mediated G1-S phase transition, thereby arresting the cell cycle in the G1 phase, suppressing DNA synthesis and inhibiting cancer cell growth. Overexpression of CDK4/6, as seen in certain types of cancer, causes cell cycle deregulation.

 

Indikationen gemäss Compendium®:

Kisqali ist indiziert bei Hormon-Rezeptor (HR)-positivem, humanen epidermalen Wachstumsfaktor-Rezeptor-2 (HER2)-negativem fortgeschrittenem oder metastasierendem Mammakarzinom in Kombination:

  • mit einem Aromatasehemmer als anfängliche endokrine Therapie zur Behandlung prä-, peri-oder postmenopausaler Frauen.
  • mit Fulvestrant als anfängliche oder nach einer vorherigen endokrinen Therapie zur Behandlung postmenopausaler Frauen.

Bei prä- oder perimenopausalen Frauen sollte die endokrine Therapie mit einem Agonisten des luteinisierenden Hormon-Releasing-Hormons (LHRH) kombiniert werden.

Ripretinib – Qinlock (USA)

According to the NCI website, ripretinib is an orally bioavailable switch pocket control inhibitor of wild-type and mutated forms of the tumor-associated antigens (TAA) mast/stem cell factor receptor (SCFR) KIT and platelet-derived growth factor receptor alpha (PDGFR-alpha; PDGFRa), with potential antineoplastic activity. Upon oral administration, ripretinib targets and binds to both wild-type and mutant forms of KIT and PDGFRa specifically at their switch pocket binding sites, thereby preventing the switch from inactive to active conformations of these kinases and inactivating their wild-type and mutant forms. This abrogates KIT/PDGFRa-mediated tumor cell signaling and prevents proliferation in KIT/PDGFRa-driven cancers. DCC-2618 also inhibits several other kinases, including vascular endothelial growth factor receptor type 2 (VEGFR2; KDR), angiopoietin-1 receptor (TIE2; TEK), PDGFR-beta and macrophage colony-stimulating factor 1 receptor (FMS; CSF1R), thereby further inhibiting tumor cell growth. KIT and PDGFRa are tyrosine kinase receptors that are upregulated or mutated in a variety of cancer cell types; mutated forms play a key role in the regulation of tumor cell proliferation and resistance to chemotherapy. Indikationen/Anwendungsmöglichkeiten gemäss NCI Website:

  • Gastrointestinal stromal tumor that is advanced. It is used in adults whose cancer has been treated with at least three kinase inhibitors, including imatinib mesylate.
  • Ripretinib is also being studied in the treatment of other types of cancer.

Rucaparib  Rubraca® (USA, EU)

According to the NCI website, rucaparib camsylate is an inhibitor of the nuclear enzyme polyadenosine 5’-diphosphoribose (poly-ADP-ribose) polymerase (PARP), with chemosensitizing, radiosensitizing, and antineoplastic activities. Upon administration, rucaparib selectively binds to PARP-1, PARP-2 and PARP-3, and inhibits PARP1-mediated repair of single-strand DNA (ssDNA) breaks via the base-excision repair pathway; this enhances the accumulation of DNA strand breaks and promotes genomic instability and apoptosis. Rucaparib may potentiate the cytotoxicity of DNA-damaging agents and reverse tumor cell resistance to chemotherapy and radiation therapy. PARP catalyzes post-translational ADP-ribosylation of nuclear proteins and is activated by ssDNA breaks. Check for active clinical trials using this agent. (NCI Thesaurus)

Rubraca ist indiziert als Erhaltungstherapie bei erwachsenen Patientinnen mit fortgeschrittenem, platinsensitivem, rezidiviertem, highgrade serösem Ovarial-, Eileiter- oder primärem Peritonealkarzinom, im Anschluss an eine platinbasierte Chemotherapie bei Vorliegen einer vollständigen oder partiellen Remission.

Ruxolitinib - JAKAVI®

Das Phosphatsalz von Ruxolitinib - Jakavi®, ein oral bioverfügbar Janus-assoziierter Kinase (JAK) –Inhibitor, verfügt über potentielle antineoplastische und immunmodulierende Aktivitäten. Ruxolitinib bindet spezifisch an und hemmt die Proteintyrosinkinasen JAK 1 und 2, was zu einer Verringerung der Entzündung und einer Hemmung der Zellproliferation führt. Der JAK-STAT (Signalwandler und Aktivator der Transkription) Signalweg spielt eine Schlüsselrolle bei der Signalübermittlung vieler Zytokine und Wachstumsfaktoren und ist an der zellulären Proliferation, dem Wachstum und der Hämatopoese sowie der Immunantwort beteiligt; JAK-Kinasen können bei entzündlichen und myeloproliferative sowie verschiedenen bösartigen Erkrankungen hochreguliert werden.

Myelofibrose (MF)

  • Zur Behandlung der Splenomegalie oder krankheitsassoziierter Symptome bei Patienten mit Myelofibrose intermediären oder hohen Risikos, mit primärer Myelofibrose oder als Komplikationen einer Polycythaemia vera oder Essentiellen Thrombozythämie.

Polycythaemia vera (PV)

  • Zur Behandlung von Patienten mit Polycythaemia vera, die gegenüber einer Behandlung mit Hydroxyurea oder einer anderen zytoreduktiven Erstlinientherapie resistent sind oder diese nicht vertragen.
