Vandetanib - CAPRELSA®

Vandetanib - Caprelsa® ist ein oral bioverfügbares 4-Anilinochinazolin. Vandetanib hemmt selektiv die Tyrosinkinase-Aktivität des vaskulären endothelialen Wachstumsfaktor-Rezeptors 2 (VEGFR2), wodurch die VEGF-stimulierte Endothelzellenproliferation und -migration blockiert und die Tumorgefäßdurchlässigkeit verringert wird. Dieses Mittel blockiert auch die Tyrosinkinase-Aktivität des epidermalen Wachstumsfaktor-Rezeptors (EGFR), eine Rezeptor- Tyrosin-Kinase, die die Tumorzellproliferation und -migration sowie Angiogenese vermittelt.

Indikationen/Anwendungsmöglichkeiten gemäss Compendium®:
Zur Behandlung von Patienten mit nicht resezierbarem, schnell fortschreitendem und symptomatischem, metastasiertem medullärem Schilddrüsenkarzinom.

Vemurafenib - ZELBORAF®
Vemurafenib - Zelboraf® ist ein oral bioverfügbarer, ATP-kompetitiver, niedermolekularer Inhibitor der BRAF (V600E) Kinase mit einer potentiellen antineoplastischen Aktivität. Vemurafenib bindet selektiv an die ATP-Bindungsstelle der BRAF (V600E) Kinase und inhibiert seine Aktivität.  Dies kann zu einer Hemmung des überaktivierten MAPK Signalwegs stromabwärts von BRAF (V600E) kinase-exprimierenden Tumorzellen führen sowie zu einer Reduktion der Tumorzellvermehrung. Etwa 90% der BRAF - Gen - Mutationen beinhalten eine Valin-zu-Glutaminsäure - Mutation am Rest 600 (V600E); das Onkogen Proteinprodukt  BRAF (V600E) Kinase zeigt eine deutlich erhöhte Aktivität, die den MAPK-Signalweg überaktiviert. Die BRAF (V600E) Genmutation findet sich in etwa 60% der Melanome und in etwa 8% aller soliden Tumoren, einschließlich Melanom, Darm-, Schilddrüsen- und andere Krebsarten.
 
Indikationen/Anwendungsmöglichkeiten gemäss Compendium®:
Behandlung von Patienten mit nicht resezierbaren oder metastasierten Melanom mit einer BRAF V600 Mutation.

Venetoclax - VENCLYXTO®

According to the NCI website venetoclax is an orally bioavailable, selective small molecule inhibitor of the anti-apoptotic protein Bcl-2, with potential antineoplastic activity. Venetoclax mimics BH3-only proteins, the native ligands of Bcl-2 and apoptosis activators, by binding to the hydrophobic groove of Bcl-2 proteins thereby repressing Bcl-2 activity and restoring apoptotic processes in tumor cells. Bcl-2 protein is overexpressed in some cancers and plays an important role in the regulation of apoptosis; its expression is associated with increased drug resistance and tumor cell survival. Compared to the Bcl-2 inhibitor navitoclax, this agent does not inhibit bcl-XL and does not cause bcl-XL-mediated thrombocytopenia.

 

Indikationen/Anwendungsmöglichkeiten gemäss Compendium®:

  • Venclyxto wird in Kombination mit Rituximab angewendet bei Erwachsenen zur Behandlung von chronisch lymphatischer Leukämie (CLL), die mindestens eine Vortherapie erhalten haben.
  • Venclyxto wird als Monotherapie angewendet bei Erwachsenen zur Behandlung einer CLL, die eine 17p‑Deletion oder TP53-Mutation aufweisen und die unter einer Behandlung mit einem Inhibitor des B‑Zell-Rezeptor-Signalwegs ein Therapieversagen zeigten.

Vinorelbine - NAVELBINE® sowie Generika

Das Ditartrat Salz eines halbsynthetischen Vinca-Alkaloids wird aus den Blättern der Pflanze Immergrün (Vinca rosea) gewonnen und hat eine antineoplastische Aktivität. Vinorelbin bindet an Tubulin, wodurch die Tubulin-Polymerisation zu Mikrotubuli und die Spindelbildung gehemmt wird. Dies führt zu einer Apoptose der dafür anfälligen Krebszellen. Die Hemmung der mitotischen Mikrotubuli korreliert mit der Anti-Tumor-Aktivität, wogegen die Hemmung der axonalen Mikrotubuli durch Vinorelbin mit der Neurotoxizität zu korrelieren scheint. Im Vergleich zu verwandten Vinca-Alkaloiden, ist Vinorelbin selektiver gegen mitotische als axonale Mikrotubuli in vitro, was seine verringerte Neurotoxizität erklärt. Vinorelbin ist auch ein Strahlungssensibilisator.

Indikationen/Anwendungsmöglichkeiten gemäss Compendium®:

  • Behandlung des nicht-kleinzelligen Bronchialkarzinoms in Kombination mit einem Platinderivat (z.B. Cisplatin).
  • Behandlung des fortgeschrittenen oder metastasierenden Mammakarzinoms in Monotherapie oder in Kombination mit Capecitabin nach Versagen einer Therapie mit Anthrazyklinen und Taxanen.

Vismodegib - ERIVEDGE®

Vismodegib ist ein oral bioverfügbares kleines Molekül mit einer potentiellen antineoplastischen Aktivität. Vismodegib zielt auf den Signalweg Hedgehog und blockiert die Aktivitäten der Hedgehog-Ligand-Zelloberflächenrezeptoren PTCH und/oder SMO und unterdrückt die Hedgehog-Signalisierung. Der Hedgehog-Signalweg spielt eine wichtige Rolle beim Gewebewachstum und der Reparatur; eine anomale konstitutive Aktivierung des Hedgehog-Signalwegs sowie die unkontrollierte Zellproliferation kann mit Mutationen in den Hedgehog-Ligand-Zelloberflächenrezeptoren PTCH und SMO in Verbindung gebracht werden.

Indikationen/Anwendungsmöglichkeiten gemäss Compendium®:

  • Erivedge ist indiziert zur Behandlung von erwachsenen Patienten mit fortgeschrittenem Basalzellkarzinom, für die eine chirurgische Behandlung oder Strahlentherapie nicht in Betracht kommt.

Vorasidenib

According to the NCI website, the IDH mutant inhibitor AG-881 (Vorasidenib) is an orally available inhibitor of mutated forms of both isocitrate dehydrogenase type 1 (IDH1, IDH1 [NADP+] soluble) in the cytoplasm and type 2 (IDH2, isocitrate dehydrogenase [NADP+], mitochondrial) in the mitochondria, with potential antineoplastic activity. Upon administration, pan-IDH mutant inhibitor AG-881 specifically inhibits mutant forms of IDH1 and IDH2, thereby inhibiting the formation of the oncometabolite 2-hydroxyglutarate (2HG) from alpha-ketoglutarate (a-KG). This prevents 2HG-mediated signaling and leads to both an induction of cellular differentiation and an inhibition of cellular proliferation in tumor cells expressing IDH mutations. In addition, AG-881 is able to penetrate the blood-brain barrier (BBB). IDH1 and 2, metabolic enzymes that catalyze the conversion of isocitrate into a-KG, play key roles in energy production and are mutated in a variety of cancer cell types. In addition, mutant forms of IDH1 and 2 catalyze the formation of 2HG and drive cancer growth by blocking cellular differentiation and inducing cellular proliferation.
