Dabrafenib - TAFINLAR®

Dabrafenib - Tafinlar® ist ein oral bioverfügbarer Inhibitor des B-Raf (BRAF) Proteins mit einer potentiellen antineoplastischen Aktivität. Dabrafenib bindet selektiv an B-Raf und hemmt die Aktivität von B-Raf, beziehungsweise die Proliferation von Tumorzellen, die ein mutiertes BRAF-Gen enthalten. B-raf gehört zur raf/mil Familie der Serin/Threonin-Proteinkinasen und spielt eine Rolle bei der Regulierung des MAP-Kinase/ERK-Signalwegs, der aufgrund der BRAF-Gen-Mutationen aktiviert werden kann.

Indikationen/Anwendungsmöglichkeiten gemäss Fachinformation des Compendium®:

Nicht resezierbares oder metastasiertes Melanom

  • Tafinlar in Kombination mit Trametinib ist angezeigt zur Behandlung von erwachsenen Patienten mit nicht resezierbarem oder metastasiertem Melanom mit einer BRAF-V600-Mutation (V600E/K).
  • Tafinlar ist angezeigt zur Behandlung von erwachsenen chemotherapienaiven Patienten mit nicht resezierbarem oder metastasiertem Melanom mit einer BRAF-V600E-Mutation (siehe «Eigenschaften/Wirkungen»). Zur Diagnose des Vorliegens einer V600E-Mutation ist die Anwendung eines validierten BRAF-Mutationstests erforderlich.

Adjuvante Behandlung des Melanoms

Tafinlar in Kombination mit Trametinib ist angezeigt zur adjuvanten Behandlung von Patienten mit Melanom im Stadium III mit einer BRAF-V600-Mutation nach vollständiger Resektion.

Fortgeschrittenes oder metastasiertes, nicht-kleinzelliges Lungenkarzinom

Tafinlar in Kombination mit Trametinib ist angezeigt zur Behandlung von Patienten mit fortgeschrittenem oder metastasiertem, nicht-kleinzelligem Lungenkarzinom (NSCLC; non-small cell lung cancer) mit einer BRAF-V600E Mutation nach einer vorausgegangenen Chemotherapie.

According to the NCI dacarbazine is a triazene derivative with antineoplastic activity. Dacarbazine alkylates and cross-links DNA during all phases of the cell cycle, resulting in disruption of DNA function, cell cycle arrest, and apoptosis. Check for active clinical trials using this agent.

Alkylating Agents

Dacomitinib - VIZIMPRO®

According to the NCI website Dacomitinibis an orally bioavailable, highly selective, second-generation small-molecule inhibitor of the pan-epidermal growth factor receptor (EGFR) family of tyrosine kinases (ErbB family) with potential antineoplastic activity. Dacomitinib specifically and irreversibly binds to and inhibits human EGFR subtypes, resulting in inhibition of proliferation and induction of apoptosis in EGFR-expressing tumor cells. EGFRs play major roles in tumor cell proliferation and tumor vascularization, and are often overexpressed or mutated in various tumor cell types. Check for active clinical trials using this agent.

Vizimpro ist als Monotherapie zur Erstlinienbehandlung von erwachsenen Patienten mit lokal fortgeschrittenem oder metastasiertem nicht-kleinzelligen Lungenkarzinom (NSCLC) mit aktivierenden Exon 19 Deletionen oder Exon 21 (L858R) Substitutionsmutationen des epidermalen Wachstumsfaktor-Rezeptor (EGFR) indiziert.

Dactinomycin/Actinomycin D

Cosmegen®; According to the NCI dactinomycin is a chromopeptide antineoplastic antibiotic isolated from the bacterium Streptomyces parvulus. Dactinomycin intercalates between adjacent guanine-cytosine base pairs, blocking the transcription of DNA by RNA polymerase; it also causes single-strand DNA breaks, possibly via a free-radical intermediate or an interaction with topoisomerase II.

Indikationen/Anwendungsmöglichkeiten:

Cosmegen ist indiziert als Kombinationspartner einer Mehrfach-Chemotherapie bei Wilms-Tumor, kindlichem Rhabdomyosarkom, Ewing-Sarkom und metastatischem Hodenkarzinom.
Cosmegen ist indiziert als Monotherapie oder als Kombinationspartner einer Mehrfach-Chemotherapie bei Chorionkarzinom.
Cosmegen kann als Komponente der regionalen Perfusionstherapie in Kombination mit Melphalan bei lokalisierten, wiederholtem oder lokalisiertem metastatischem malignem Melanom angewendet werden.

Es wurde in der Schweiz im Jahr 2012 laut PharmaWiki aus wirtschaftlichen Gründen vom Markt genommen.

Zytostatikum

Daratumumab - DARZALEX®

According to the NCI website Daratumumab is a fully human monoclonal antibody directed against the cell surface glycoprotein CD-38 with potential antineoplastic activity. The binding of daratumumab to natural killer (NK) cells mimics the normal CD38-CD31 interaction on the NK cell surface. CD38 is also present on multiple myeloma (MM) cells and plasma leukemia cells; this agent may preferentially bind these cells, triggering antitumoral antibody-dependent cellular cytotoxicity (ADCC) and complement-dependent cytotoxicity (CDC). CD38, a cell surface glycoprotein, is present on various immune cells and has been shown to regulate the cytotoxic response of activated NK cells.

Indikationen/Anwendungsmöglichkeiten gemäss Fachinformation des Compendium®:

  • DARZALEX ist indiziert als Monotherapie für die Behandlung von Patienten mit multiplem Myelom, die mindestens drei frühere Therapielinien erhalten haben, einschliesslich >1 Proteasomen-Inhibitors (PI) und >1 immunmodulatorischen Wirkstoffs (IMiD) oder welche gegenüber >1 PI und IMiD doppel-refraktär waren.
  • DARZALEX ist indiziert in Kombination mit Lenalidomid und Dexamethason oder Bortezomib und Dexamethason für die Behandlung von Patienten mit multiplem Myelom, die mindestens eine frühere Therapielinie erhalten haben.
  • DARZALEX ist indiziert in Kombination mit Lenalidomid und Dexamethason oder mit Bortezomib, Melphalan und Prednison für die Behandlung von bisher unbehandelten Patienten mit multiplem Myelom, die für eine autologe Stammzelltransplantation nicht geeignet sind.

Darbepoetin - ARANESP®

According to the NCI Darbepoetin alfa A recombinant analog of the endogenous cytokine erythropoietin, an erythropoiesis-stimulating protein. Due to the addition of two carbohydrate chains, darbepoetin alfa exhibits a three-fold greater half-life than does erythropoietin. Similar to erythropoietin, darbopoietin alfa binds to and activates epoetin receptors, thereby inducing the differentiation and maturation of erythrocyte progenitors, stimulating endothelial cell proliferation, and stimulating B-cell proliferation and immunoglobulin production.

Indikationen/Anwendungsmöglichkeiten gemäss Compendium®:

  • Zur Behandlung der symptomatischen Anämie bei chronischer Niereninsuffizienz bei erwachsenen und pädiatrischen Patienten ab 1 Jahr.
  • Zur Behandlung der symptomatischen Anämie bei erwachsenen Krebspatienten mit nicht-myeloischen malignen Erkrankungen und unter einer Chemotherapie.
  • Zur Behandlung der Anämie bei erwachsenen Patienten mit geringem Transfusionsbedarf bei myelodysplastischen Syndromen der Risikogruppen low oder intermediate-1 und niedrigen endogenen Erythropoetin-Spiegeln.

Erythropoiesis-Stimulating Agent

Dasatinib - SPRYCEL®
Dasatinib ist ein oral bioverfügbarer synthetischer niedermolekulare Inhibitor der SRC-Familie von Protein-Tyrosin-Kinasen. Dasatinib bindet an und hemmt die wachstumsfördernden Aktivitäten dieser Kinasen. Offensichtlich wegen der weniger strengen Bindungsaffinität für die BCR-ABL-Kinase wurde gezeigt, dass Dasatinib die Resistenz der in CML-Zellen enthaltenen BCR-ABL Kinasedomäne-Punktmutationen gegenüber Imatinib  zu überwinden vermag. Die zur SRC-Familie gehörenden Protein-Tyrosin-Kinasen wirken mit einer Vielzahl von Zelloberflächenrezeptoren und nehmen an den intrazellulären Signaltransduktionswegen teil;
 

Indikationen/Anwendungsmöglichkeiten gemäss Arzneimittel-Kompendium der Schweiz®:

Behandlung von Patienten mit:

  • Neu diagnostizierter Philadelphiachromosom-positiver chronischer myeloischer Leukämie (Ph+CML) in der chronischen Phase.
  • Ph+CML in der chronischen Phase bei Progression oder Resistenz auf optimale Imatinib-Dosierung, in der akzelerierten Phase oder Blastenkrise bei Progression oder Resistenz auf Imatinib.
  • Ph+CML in der chronischen Phase, der akzelerierten Phase oder Blastenkrise bei signifikanter Toxizität unter Therapie mit Imatinib.
  • Philadelphiachromosom-positiver akuter lymphatischer Leukämie (Ph+ALL) nach Resistenz oder Progression oder bei Intoleranz unter einer Kombinationsbehandlung mit Imatinib und Chemotherapie. Patienten mit bekannten T315I Mutationen sollten nicht behandelt werden.
  • Als Resistenz gegenüber Imatinib wird Nichterreichen eines hämatologischen Ansprechens (innerhalb von 3 Monaten) oder eines zytogenetischen Ansprechens (komplettes oder partielles Ansprechen innerhalb von 12 Monaten) unter Therapie mit optimaler Imatinib-Dosierung bzw. der Verlust eines kompletten hämatologischen oder zytogenetischen Ansprechens oder das Auftreten einer resistenten Mutation der BCR-ABL-Kinase definiert.

Daunorubicin

According to NCI website Daunorubicin is the hydrochloride salt of an anthracycline antineoplastic antibiotic with therapeutic effects similar to those of doxorubicin. Taxane

Dovitinib

According to the NCI website Dovitinib is an orally bioavailable lactate salt of a benzimidazole-quinolinone compound with potential antineoplastic activity. Dovitinib strongly binds to fibroblast growth factor receptor 3 (FGFR3) and inhibits its phosphorylation, which may result in the inhibition of tumor cell proliferation and the induction of tumor cell death. In addition, this agent may inhibit other members of the RTK superfamily, including the vascular endothelial growth factor receptor; fibroblast growth factor receptor 1; platelet-derived growth factor receptor type 3; FMS-like tyrosine kinase 3; stem cell factor receptor (c-KIT); and colony-stimulating factor receptor 1; this may result in an additional reduction in cellular proliferation and angiogenesis, and the induction of tumor cell apoptosis. The activation of FGFR3 is associated with cell proliferation and survival in certain cancer cell types. Check for active clinical trials using this agent.

Tyrosin Kinase Inhibitor

Doxorubicin - ADRIBLASTIN® RD/- Solution, CAELYX® sowie Generika.

According to the NCI website Doxorubicin intercalates between base pairs in the DNA helix, thereby preventing DNA replication and ultimately inhibiting protein synthesis. Additionally, doxorubicin inhibits topoisomerase II which results in an increased and stabilized cleavable enzyme-DNA linked complex during DNA replication and subsequently prevents the ligation of the nucleotide strand after double-strand breakage. Doxorubicin also forms oxygen free radicals resulting in cytotoxicity secondary to lipid peroxidation of cell membrane lipids; the formation of oxygen free radicals also contributes to the toxicity of the anthracycline antibiotics, namely the cardiac and cutaneous vascular effects.

Indikationen/Anwendungsmöglichkeiten gemäss Compendium®:

Caelyx®

  • Behandlung von Patientinnen mit metastasierendem Mammakarzinom, wenn eine Monotherapie mit Anthrazyklinen geeignet ist.
  • Behandlung des fortgeschrittenen Ovarialkarzinoms bei Versagen/Rezidiv nach einer Paclitaxel- und/oder Platin-haltigen Chemotherapie.
  • In Kombination mit Bortezomib für die Behandlung des progressiven multiplen Myeloms bei Patienten, die mindestens eine vorangehende Therapie erhalten haben und die sich bereits einer Knochenmarktransplantation unterzogen haben oder die für eine Knochenmarktransplantation nicht geeignet sind.
  • Behandlung des AIDS-assoziierten Kaposi-Sarkoms bei Patienten mit niedrigen CD4-Werten (<200 CD4-Lymphozyten/µl) und ausgedehnter mucokutaner oder viszeraler Beteiligung.
  • Caelyx kann als systemische First-Line Chemotherapie oder als Second-Line Chemotherapie bei AIDS-Kaposi-Patienten bei Progredienz der Erkrankung oder Intoleranz unter vorausgehender systemischer Kombinationstherapie mit mindestens zwei der folgenden Zytostatika verwendet werden: Vinca-Alkaloid, Bleomycin und Doxorubicin (oder einem anderen Anthrazyclin).

Duligotuzumab (MEHD7945A)
According to the NCI website Duligotuzumab (MEHD7945A) is an anti-EGFR/HER3 monoclonal antibody, an immunoglobulin (Ig) G1 monoclonal antibody directed against both human epidermal growth factor receptor 3 (HER3 or ERBB3) and human epidermal growth factor receptor (EGFR), with potential antineoplastic activity. Anti-EGFR/HER3 Monoclonal Antibody MEHD7945A binds to both EGFR and HER3 and inhibits their activation. This may prevent EGFR/HER3-mediated signaling and inhibit EGFR/HER3-dependent tumor cell proliferation. In addition, MEHD7945A induces antibody-dependent cell-mediated cytotoxicity (ADCC) against EGR/HER3-expressing tumor cells. EGFR and HER3, members of the epidermal growth factor receptor (EGFR) family of receptor tyrosine kinases, are frequently overexpressed in tumors; their expression is associated with both poor prognosis and drug resistance.

Durvalumab - IMFINZI®

According to the NCI website durvalumab is a Fc optimized monoclonal antibody directed against programmed cell death-1 ligand 1 (PD-L1; B7 homolog 1; B7H1), with potential immune checkpoint inhibitory and antineoplastic activities. Upon intravenous administration, durvalumab binds to PD-L1, thereby blocking its binding to and activation of its receptor programmed death 1 (PD-1) expressed on activated T-cells. This may reverse T-cell inactivation and activate the immune system to exert a cytotoxic T-lymphocyte (CTL) response against PD-L1-expressing tumor cells. PD-L1, a member of the B7 protein superfamily, is overexpressed on certain tumor cell types and on various tumor-infiltrating immune cells. PD-L1 binding to PD-1 on T-cells suppresses the immune system and results in increased immune evasion. The Fc region of durvalumab is modified in such a way that it does not induce either antibody-dependent cytotoxicity (ADCC) or complement-dependent cytotoxicity (CDC).

  • Imfinzi ist indiziert für die Behandlung von Patienten mit lokal fortgeschrittenem, nicht resezierbarem nicht-kleinzelligem Lungenkarzinom (NSCLC), deren Erkrankung nach einer definitiven platinbasierten Chemoradiotherapie nicht fortgeschritten ist.
