Belantamab mafodotin

According to the NCI website, belantamab mafodotin is an antibody-drug conjugate (ADC) consisting of belantamab, an afucosylated, humanized monoclonal antibody, directed against the B-cell maturation antigen (BCMA), conjugated to mafodotin, an auristatin analogue and microtubule inhibitor monomethyl auristatin phenylalanine (MMAF), with potential antineoplastic activity. Upon administration of belantamab mafodotin, the anti-BCMA antibody moiety selectively binds to BCMA on tumor cell surfaces. Upon internalization, the MMAF moiety binds to tubulin and inhibits its polymerization, which results in G2/M phase arrest and induces tumor cell apoptosis. In addition, belantamab mafodotin induces antibody-dependent cellular cytotoxicity (ADCC). Altogether, this results in the inhibition of cellular proliferation in tumor cells that overexpress BCMA. BCMA, a receptor for a proliferation-inducing ligand and B-cell activating factor, is a member of the tumor necrosis factor (TNF) receptor superfamily and plays a key role in plasma cell survival; it is found on the surfaces of plasma cells and overexpressed on malignant plasma cells. Bevacizumab - AVASTIN®

Avastin® - A recombinant humanized monoclonal antibody directed against the vascular endothelial growth factor (VEGF), a pro-angiogenic cytokine.

Indikationen/Anwendungsmöglichkeiten gemäss Compendium:

Metastasiertes Kolon- oder Rektumkarzinom

Avastin (Bevacizumab) ist zur Erstlinientherapie bei Patienten mit metastasiertem Karzinom des Kolons oder Rektums in Kombination mit folgenden Chemotherapien indiziert:

  • 5-Fluorouracil/Folinsäure
  • 5-Fluorouracil/Folinsäure/Irinotecan
  • Capecitabine/Oxaliplatin (XELOX)

Avastin ist als Zweitlinientherapie in Kombination mit einem Irinotecan- oder Oxaliplatin-haltigen Chemotherapieschema bei Patienten mit metastasiertem Kolon- oder Rektumkarzinom mit vorheriger Oxaliplatin- oder Irinotecan-basierter Chemotherapie mit oder ohne Avastin indiziert.

Metastasiertes Mammakarzinom

Avastin (Bevacizumab) ist in Kombination mit Paclitaxel zur Erstlinientherapie von Patienten mit HER2-negativem, metastasiertem Mammakarzinom indiziert.

Fortgeschrittenes, metastasiertes oder rezidivierendes nicht-kleinzelliges Lungenkarzinom (NSCLC)

Erstlinientherapie in Kombination mit einer Cisplatin- und Gemcitabin-haltigen Chemotherapie bei nicht-resezierbarem, fortgeschrittenem, metastasiertem oder rezidivierendem, nicht-plattenepithelartigem, nicht-kleinzelligem Lungenkarzinom (NSCLC).

Fortgeschrittenes und/oder metastasierendes Nierenzellkarzinom

Avastin in Kombination mit Interferon alfa-2a ist zur Erstlinientherapie von nephrektomierten Patienten mit fortgeschrittenem und/oder metastasierendem Nierenzellkarzinom indiziert.

Glioblastom (WHO Grad IV)

Avastin ist als Monotherapie zur Behandlung von Patienten mit rezidivierendem Glioblastom (WHO Grad IV) nach Vortherapie mit Temozolomid indiziert.

Ovarialkarzinom

Nicht-vorbehandelte Patienten

Avastin ist in Kombination mit Carboplatin und Paclitaxel für die Therapie nicht vorbehandelter Patienten mit Ovarialkarzinom (FIGO Stadium III und IV), bei welchen der Tumor nicht vollständig reseziert werden konnte und welche von einer im Anschluss an die Chemotherapie durchzuführenden weiteren Operation (Second look mit interval debulking) nicht potenziell profitieren, indiziert.

Rezidivierende, platin-sensitive Patienten

Avastin in Kombination mit Carboplatin und Gemcitabin oder in Kombination mit Carboplatin und Paclitaxel ist zur Behandlung von Patienten mit rezidivierendem, platin-sensitivem epithelialem Ovarialkarzinom, Eileiterkarzinom oder primärem Peritonealkarzinom indiziert (mit einem platinfreien Intervall von mindestens 6 Monaten).

Rezidivierende, platin-resistente Patienten

Avastin ist indiziert in Kombination mit Paclitaxel, Topotecan oder pegyliertem liposomalem Doxorubicin zur Behandlung von Patienten mit rezidivierendem, platin-resistentem epithelialem Ovarialkarzinom, Eileiterkarzinom oder primärem Peritonealkarzinom, die mit bis zu zwei vorgängigen Chemotherapieregimen behandelt wurden, und nicht vorbehandelt wurden mit Bevacizumab oder anderen VEGF-Inhibitoren.

Zervixkarzinom

Avastin ist in Kombination mit Chemotherapie (siehe «Eigenschaften/Wirkungen; Zervixkarzinom») für die Behandlung von Patienten mit persistierendem, rezidivierendem oder metastasiertem Zervixkarzinom indiziert.

Bexarotene

According to the NCI bexarotene is a synthetic retinoic acid agent with potential antineoplastic, chemopreventive, teratogenic and embryotoxic properties. Bexarotene selectively binds to and activates retinoid X receptors (RXRs), thereby inducing changes in gene expression that lead to cell differentiation, decreased cell proliferation, apoptosis of some cancer cell types, and tumor regression.

Indikationen/Anwendungsmöglichkeiten gemäss Chemocare.com:
Cutaneous T-cell lymphoma (mycosis fungoides) in patients who have not responded to at least one previous treatment regimen.

Bicalutamid - Casodex® sowie diverse Generika

Bicalutamid ist ein synthetisches nicht steroidales Antiandrogen. Es bindet sich kompetitiv an den cytosolischen Androgenrezeptor im Zielgewebe und hemmt dadurch die Bindung der Androgene an den Androgenrezeptor. Keine Verbindung findet statt mit den meisten mutierten Formen von Androgenrezeptoren.

 

Indikationen/Anwendungsmöglichkeiten gemäss Arzneimittelkompendium:

  • Casodex (50 mg/Tag) in Kombination mit LHRH-Analoga oder Orchiektomie beim fortgeschrittenen, metastasierenden Prostatakarzinom (M1). Die Erfahrungen in Kombination mit chirurgischer Kastration sind beschränkt.
  • Casodex (150 mg/Tag) beim lokal fortgeschrittenen Prostatakarzinom (T3–4, jedes N, M0; T1–2, N , M0) als Monotherapie, wenn die Standardtherapien radikale Prostatektomie, Radiotherapie, chirurgische oder medikamentöse Kastration nicht geeignet sind.
  • Casodex (150 mg/Tag) adjuvant nach radikaler Prostatektomie oder Strahlentherapie bei Patienten mit einem lokal fortgeschrittenen Prostatakarzinom und hohem Risiko einer Progression.
  • Beim metastasierenden Prostatakarzinom soll die Monotherapie mit Casodex 150 mg/Tag nicht angewendet werden

Antiandrogen

Bintrafusp alfa

According to the NCI website, bintrafusp alfa is a  bifunctional fusion protein composed of avelumab, an anti-programmed death ligand 1 (PD-L1) human monoclonal antibody, bound to the soluble extracellular domain of human transforming growth factor beta (TGFbeta) receptor type II (TGFbetaRII), with potential antineoplastic and immune checkpoint modulating activities. Upon administration, the TGFbetaRII moiety of bintrafusp alfa binds to and neutralizes TGFbeta while the avelumab moiety simultaneously binds to PD-L1. This prevents TGFbeta- and PD-L1-mediated signaling, and increases natural killer (NK) cell and cytotoxic T-lymphocyte (CTL) activities. This inhibits tumor cell proliferation in susceptible tumor cells. TGFbeta and PD-L1 are both upregulated in certain types of cancers; their overexpression is associated with increased evasion of immune surveillance and contributes to poor prognosis. Bleomycin

Bleomycin Baxter, Bleomycin-Teva®; According to the NCI website Bleomycinsulfate forms complexes with iron that reduce molecular oxygen to superoxide and hydroxyl radicals which cause single- and double-stranded breaks in DNA; these reactive oxygen species also induce lipid peroxidation, carbohydrate oxidation, and alterations in prostaglandin synthesis and degradation.

IIndikationen/Anwendungsmöglichkeiten gemäss Arzneimittelkompendium:
Plattenepithel-Karzinom, besonders im Bereich von Mund, Pharynx, Nasen-Nebenhöhlen, Larynx, Ösophagus, Bronchien, Zervix, Vagina, Penis und Haut. Gut differenziert Tumoren reagieren besser als anaplastische Formen.

Blinatumomab - BLINCYTO®

According to the NCI website blinatumomab is a recombinant, single-chain, anti-CD19/anti-CD3 bispecific monoclonal antibody with potential immunostimulating and antineoplastic activities. Blinatumomab posesses two antigen-recognition sites, one for the CD3 complex, a group of T cell surface glycoproteins that complex with the T cell receptor (TCR), and one for CD19, a tumor-associated antigen (TAA) overexpressed on the surface of B cells. This bispecific monoclonal antibody brings CD19-expressing tumor B-cells and cytotoxic T lymphocytes (CTLs) and helper T lymphocytes (HTLs) together, which may result in the CTL- and HTL-mediated cell death of CD19-expressing B-lymphocytes.

Indikationen gemäss Compendium

  • BLINCYTO ist indiziert für die Behandlung von Patienten mit einer rezidivierenden oder refraktären Philadelphia-Chromosom-negativen B‑Vorläufer-ALL (akute lymphoblastische Leukämie).
  • BLINCYTO kann angewendet werden bei Patienten mit B-Vorläufer-ALL in kompletter Remission mit MRD (minimal residual disease) ≥0.1%.

Bortezomib - VELCADE® sowie Generika

Velcade® - A dipeptide boronic acid analogue with antineoplastic activity.

Indikationen/Anwendungsmöglichkeiten gemäss Arzneimittelkompendium:

Multiples Myelom

  • In Kombination mit Melphalan und Prednison bei bisher unbehandelten Patienten mit multiplem Myelom.
  • In Kombination mit Standardchemotherapie und Stammzelltransplantation bei erwachsenen Patienten mit multiplem Myelom.
  • Behandlung von Patienten mit rezidivierendem/refraktärem multiplem Myelom, welche mindestens eine vorhergehende Therapie erhielten.

Mantelzell-Lymphom

  • In Kombination mit Rituximab, Cyclophosphamid, Doxorubicin und Prednison zur Behandlung von erwachsenen Patienten mit vorgängig unbehandeltem Mantelzell-Lymphom, die für eine hämatopoetische Stammzelltransplantation nicht geeignet sind.
  • Behandlung von Patienten mit rezidivierendem/refraktärem Mantelzell-Lymphom Stadium III/IV, welche mindestens eine vorhergehende Therapie erhielten und bei welchen vor Beginn der Therapie mit Bortezomib eine Translokation t (11; 14) (q13; q32) oder Überexpression von Cyclin D1 nachgewiesen wurde. Behandelt werden sollten nur Patienten, bei welchen eine Stammzelltransplantation nicht durchführbar ist.

Proteasome Inhibitor

Bosutinib - Bosulif®

Bosutinib ist ein synthetisches Quinolon-Derivat und ein dualer inase-Hemmer der sowohl auf die  Abl- und auch die Src-Kinases abziehlt. Dies führt zu einer potentiell antineoplastischen Aktivität. Nicht wie Imatinib hemmt Bosutinib die Autophosphorylisierung der Abl- und Src-Kinasen.  Dies führt zu einer Hemmung des Zellwachstums sowie zur Apoptose. Aufgrund der dualen Wirkungsweise kann das Medikament eine Wirkung bei der resistenten CML entfalten sowie auch bei anderen myeloiden Erkrankungen sowie bei soliden Tumoren. 

Indikationen/Anwendungsmöglichkeiten gemäss Fachinformation des Arzneimittel-Kompendium der Schweiz®:

  • Bosulif ist indiziert für die Behandlung von erwachsenen Patienten mit neu diagnostizierter Philadelphia-Chromosom positiver chronischer myeloischer Leukämie (Ph+ CML) in der chronischen Phase.
  • Bosulif ist indiziert für die Behandlung von erwachsenen Patienten mit Philadelphia-Chromosom positiver chronisch myeloischer Leukämie (Ph+ CML) in der chronischen Phase, in der akzelerierten Phase sowie in der Blastenkrise, nach Vorbehandlung mit einem oder mehreren c-abl Tyrosinkinase Inhibitoren und wenn eine Behandlung mit Imatinib, Nilotinib oder Dasatinib nicht in Frage kommt.

Brentuximab - ADCETRIS®

According to the NCI Brentuximab is an antibody-drug conjugate (ADC) directed against the tumor necrosis factor (TNF) receptor CD30 with potential antineoplastic activity.

Indikationen/Anwendungsmöglichkeiten gemäss Fachinformation Compendium.ch®:
 
  • Adcetris ist für die Therapie bei unvorbehandelten erwachsenen Patienten mit CD30 positivem (+), Stadium IV Hodgkin-Lymphom (HL) in Kombination mit einer Chemotherapie mit Doxorubicin, Vinblastin und Dacarbazin (AVD) indiziert (siehe «Klinische Wirksamkeit»).
  • Adcetris in Kombination mit AVD ist nicht mit einer BEACOPP eskalierten-Chemotherapie verglichen worden, sondern mit ABVD-Chemotherapie bestehend aus Doxorubicin, Bleomycin, Vinblastin und Dacarbazin.
  • Adcetris ist nach einer autologen Stammzelltransplantation für die Behandlung von erwachsenen Patienten mit CD30+ Hodgkin-Lymphom (HL) indiziert, die ein erhöhtes Risiko für ein Rezidiv oder Progression aufweisen (siehe «Klinische Wirksamkeit»).
  • Adcetris ist für die Behandlung von Patienten mit rezidiviertem oder refraktärem CD30+ Hodgkin-Lymphom (HL) indiziert:

- nach einer autologen Stammzell-Transplantation

- oder nach zumindest zwei vorherigen Behandlungen, falls eine Stammzell-Transplantation keine  

  Behandlungsmöglichkeit darstellt.

  • Adcetris ist für die Behandlung von Patienten mit rezidiviertem oder refraktärem systemischen anaplastischen grosszelligen Lymphom (sALCL) indiziert.
  • Adcetris ist bei erwachsenen Patienten mit CD30+ kutanem T-Zell-Lymphom (CTCL) indiziert, welche unter systemischer Therapie eine Progression zeigen oder für die eine andere systemische Therapie nicht in Frage kommt.

Brigatinib - ALUNBRIG® (USA; EU)

 

According to the NCI website, brigatinib is an orally available inhibitor of receptor tyrosine kinases anaplastic lymphoma kinase (ALK) and the epidermal growth factor receptor (EGFR) with potential antineoplastic activity. Brigatinib binds to and inhibits ALK kinase and ALK fusion proteins as well as EGFR and mutant forms. This leads to the inhibition of ALK kinase and EGFR kinase, disrupts their signaling pathways and eventually inhibits tumor cell growth in susceptible tumor cells. In addition, brigatinib appears to overcome mutation-based resistance. ALK belongs to the insulin receptor superfamily and plays an important role in nervous system development; ALK dysregulation and gene rearrangements are associated with a series of tumors. Indikationen/Anwendungsmöglichkeiten gemäss EMEA

Alunbrig is a cancer medicine that is used to treat adults with a type of lung cancer called non-small cell lung cancer (NSCLC) who have been treated before with a cancer medicine called crizotinib.

Alunbrig is used on its own and only if the NSCLC is ‘ALK-positive’, which means that the cancer cells have certain changes affecting the gene that makes a protein called ALK (anaplastic lymphoma kinase).

BST-236

According to the NCI website cytarabine-asparagine prodrug BST-236 is a small molecule pro-drug consisting of cytarabine, an antimetabolite analog of cytidine with a modified arabinose sugar moiety, covalently bonded to asparagine, with potential antineoplastic activity. Upon intravenous administration, cytarabine-asparagine prodrug BST-236 targets cancer cells, which often lack asparagine synthetase and are dependent on an external source of amino acids due to their high metabolic rate. Once the prodrug is inside target cells, the cytarabine component is cleaved and competes with cytidine for incorporation into DNA. The arabinose sugar moiety of cytarabine sterically hinders the rotation of the molecule within DNA, resulting in cell cycle arrest, specifically during the S phase of replication. Cytarabine also inhibits DNA polymerase, resulting in a decrease in DNA replication and repair. Because BST-236 specifically targets cancer cells, it may spare normal tissues from cytarabine-related toxicities. Check for active clinical trials using this agent.

Buparlisib

According to the NCI webseite Buparlisib is an orally bioavailable specific oral inhibitor of the pan-class I phosphatidylinositol 3-kinase (PI3K) family of lipid kinases with potential antineoplastic activity. Buparlisib specifically inhibits class I PIK3 in the PI3K/AKT kinase (or protein kinase B) signaling pathway in an ATP-competitive manner, thereby inhibiting the production of the secondary messenger phosphatidylinositol-3,4,5-trisphosphate and activation of the PI3K signaling pathway. This may result in inhibition of tumor cell growth and survival in susceptible tumor cell populations. Activation of the PI3K signaling pathway is frequently associated with tumorigenesis. Dysregulated PI3K signaling may contribute to tumor resistance to a variety of antineoplastic agents.

 

 

 

 

Busulfan

According to the NCI Busilvex®/Busulfan is a synthetic derivative of dimethane-sulfonate with antineoplastic and cytotoxic properties. Although its mechanism of action is not fully understood, busulfan appears to act through the alkylation of DNA. Following systemic absorption of busulfan, carbonium ions are formed, resulting in DNA alkylation and DNA breaks and inhibition of DNA replication and RNA transcription.

Indikationen/Anwendungsmöglichkeiten gemäss Arzneimittelkompendium:
Konditionierung vor einer hämatopoetischen Stammzelltransplantation.
